Precautions

This medication contains Donepezil Hydrochloride as the active ingredient and is used for the treatment of Alzheimer's-type dementia. Firstly, this medication should not be administered to certain patients. Patients who are allergic to this medication or its components, pregnant women, women who might become pregnant, and breastfeeding women should not take this medication. Additionally, patients with genetic lactose intolerance or lactase deficiency, or those with glucose-galactose malabsorption should avoid this medication due to its lactose content. Patients with heart diseases, especially those with bradycardia or heart conduction disorders, should take this medication cautiously as it may slow down heart rate or induce arrhythmia. Moreover, patients with a history of peptic ulcers or currently taking non-steroidal anti-inflammatory drugs should use this medication carefully due to the risk of gastrointestinal bleeding or exacerbation of ulcers. Patients with asthma or chronic obstructive pulmonary disease should be careful as bronchial constriction or increased mucus secretion may worsen their symptoms. Patients with extrapyramidal disorders such as Parkinson’s disease should use this medication cautiously, as it may exacerbate their symptoms. This medication contains Yellow 4 which may cause allergic reactions in some patients. While taking this medication, various adverse reactions may occur. Cardiac-related adverse effects such as syncope, bradycardia, heart block, myocardial infarction have been reported, along with gastrointestinal bleeding or ulcers, liver function impairment, cerebral hemorrhage, extrapyramidal disorder symptoms, neuroleptic malignant syndrome, rhabdomyolysis, respiratory distress, acute pancreatitis, acute renal failure, and unexplained sudden death. Additionally, various symptoms such as nausea, vomiting, diarrhea, loss of appetite, dizziness, insomnia, hallucinations, aggressive behavior, depression, drowsiness, rash, muscle pain, urinary incontinence have been reported. Most of these adverse reactions are mild to moderate, although some may require discontinuation of treatment. Notably, gastrointestinal side effects like nausea and vomiting become more frequent with increased dosage, so patients should be carefully monitored at the beginning of therapy or when increasing the dose. This medication is not approved for other types of dementia or memory disorders outside of Alzheimer's-type dementia and must be used under the diagnosis and supervision of an experienced physician. The therapeutic effect should be regularly assessed, and discontinuation should be considered if there is no effect. This medication may affect heart rate, potentially causing bradycardia or arrhythmia, thus patients with heart diseases or electrolyte imbalances may require ECG monitoring. Additionally, due to increased gastric acid secretion, there is a risk of peptic ulcer or gastrointestinal bleeding, so patients with related histories should be vigilant about their symptoms. This medication may affect the ability to drive or operate machinery, so caution should be exercised during initial intake or dose changes, as fatigue or dizziness may occur. Pregnant or breastfeeding women are advised against using this medication, and if absolutely necessary, careful consideration is required. This medication may interact with multiple drugs, especially those affecting the CYP3A4 or CYP2D6 enzymes, so caution is needed when taken in conjunction with these. Also, combinations with drugs that affect heart conduction may increase the risk of side effects, hence one should follow the physician's instructions. In case of overdose, severe nausea, vomiting, bradycardia, hypotension, respiratory depression, seizures may present. Immediate medical attention is required to receive appropriate treatment. This medication should be kept out of reach of children, and transferring it to another container is not advisable for quality maintenance.
