Improvement of blood sugar control in type 2 diabetes
This medication is used alongside diet and exercise to help control blood sugar levels in patients with type 2 diabetes. It can also be administered alone, and if blood sugar control is inadequate when starting diabetes treatment, it can be combined with metformin. Additionally, when blood sugar control is difficult with monotherapy of sulfonylureas, metformin, thiazolidinediones, or insulin, this medication can be added for combination therapy. Furthermore, for patients whose blood sugar control is insufficient, even after combining multiple medications, additional combination therapy including this medication is recommended. It is used in various scenarios to improve patients' blood sugar management.
The usual dose is 100 mg once daily for either monotherapy or combination therapy, with a maximum daily dose of 100 mg. It can be taken regardless of meals. Patients with renal impairment may require dosage adjustments based on kidney function. For patients with moderate renal impairment (glomerular filtration rate 30-45 mL/min/1.73 m2), the dose is 50 mg once daily. For patients with severe renal impairment (glomerular filtration rate 15-30 mL/min/1.73 m2) or end-stage renal disease requiring dialysis (glomerular filtration rate < 15 mL/min/1.73 m2), the dose should be adjusted accordingly.
This medication can cause rare but serious allergic reactions. Symptoms such as anaphylaxis, angioedema, and severe skin disorders may occur, and if such symptoms are suspected, it is important to stop taking the medication immediately and consult a doctor. During the administration of this medication, pancreatitis, which is inflammation of the pancreas, may occur, so if symptoms such as abdominal pain, vomiting, or nausea appear, the healthcare provider should be notified immediately. Patients with type 1 diabetes or diabetic ketoacidosis should not use this medication, and patients who have had severe allergic reactions to this medication or its ingredients should also avoid it. Patients with impaired kidney function may have delayed excretion of this medication, so dosage adjustments may be necessary, and regular checks of kidney function are important. This medication has the potential to cause hypoglycemia when used alone or in combination with other diabetes medications. Extra caution is required, especially when used with insulin or sulfonylureas, as the risk of hypoglycemia increases, and dosage adjustments for other medications may be needed if necessary. If severe joint pain or skin blistering occurs during administration, notify the doctor immediately and discontinue use. This medication is not recommended for women who are pregnant or breastfeeding, and the safety and efficacy in children and adolescents under 18 years of age have not been established. Elderly patients may have decreased kidney function, so kidney function tests should be conducted before and after taking this medication, and dosage adjustments may be needed. In cases of overdose, general emergency measures should be taken, and some can be removed through dialysis, so proper medical intervention is required. This medication has relatively few drug interactions, but efficacy or side effects may vary when used in combination with certain drugs, so all medications taken should be reported to the doctor or pharmacist.
