Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), reduction of mortality risk post-myocardial infarction
This medication is used to control essential hypertension. It also helps treat heart failure patients who are intolerant to ACE inhibitors (mainly NYHA class II to IV). It is effective in reducing the risk of death in patients with left ventricular dysfunction or impaired contractility after myocardial infarction. Thus, it improves patients' heart health by managing hypertension, heart failure, and post-myocardial infarction.
1. Essential Hypertension - Adults: Recommended oral dose of 80 mg once daily - If blood pressure control is difficult, the dose can be increased up to 160 mg per day - Maximum dose can be increased up to 320 mg per day - Elderly patients (75 years and older): Start with 40 mg once daily - Dosage adjustment according to renal and hepatic impairment 2. Heart Failure - Recommended oral dose of 40 mg twice daily - Can increase to 80 mg or 160 mg per dose depending on the patient's situation.
Safety and efficacy have not been established in children and adolescents under 18 years of age
This medication can cause serious harm to the fetus during pregnancy, especially in the second and third trimesters, so it should be discontinued immediately upon confirmation of pregnancy. Patients with allergies to this medication or its components, pregnant or breastfeeding women, patients with severe liver disease, hereditary angioedema, and patients with severe kidney impairment should not take this medication. Care should be taken when prescribing to elderly patients, patients with hyperkalemia, mild to moderate liver impairment, cardiovascular diseases, or those with sodium or fluid depletion, starting with a low dose and closely monitoring their condition. Blood pressure may drop rapidly on first use, so if symptoms of dizziness or fainting occur, medical consultation should be sought immediately. This medication can increase potassium levels in the blood, so caution is needed when taken with potassium-sparing diuretics or potassium supplements. Interactions may occur when combined with other antihypertensives, diuretics, or anti-inflammatory analgesics, so patients must inform their physician or pharmacist. If symptoms such as muscle pain, rash, difficulty breathing, severe dizziness, blood pressure drop, or abnormal kidney function occur during treatment, the medication should be stopped immediately, and medical attention should be sought. The safety and efficacy of this medication have not been established in children and adolescents, and it is not used in these populations. Elderly patients are at risk of stroke and other complications due to excessive blood pressure lowering; thus, they should start at a low dose and closely monitor their condition. In case of overdose, symptoms such as hypotension, decreased consciousness, and shock may occur; immediate emergency treatment is necessary, and blood dialysis is ineffective. Keep out of reach of children and do not transfer to another container for safety.
Angioedema (swelling of the face, lips, pharynx, tongue), severe allergic reactions, severe rash, blisters, skin peeling, pain in mucous membranes of the mouth/eyes, severe skin adverse reactions, difficulty breathing, asthma attacks, respiratory abnormalities accompanied by cough and fever, jaundice of the skin or eyes, liver abnormalities (jaundice), sudden and severe headaches, lightning pain, seizures, confusion of consciousness, visual disturbances, paralysis, severe neurological symptoms, significantly reduced urine output
