Summary

Main use
Improvement of essential hypertension
Who takes it
Adults · Children
Adult dose
The recommended initial dose for adults is 10 mg once daily, irrespective of meals. If blood pressure is not adequately controlled at this dose, it may be increased to 20 mg once daily. If further reduction in blood pressure is needed, it may be increased to a maximum of 40 mg once daily. The antihypertensive effect appears within 2 weeks of starting treatment, with maximum effect observed in about 8 weeks. If blood pressure is not adequately controlled by this medication alone, it may be used in combination with thiazide diuretics such as hydrochlorothiazide. For improved adherence, it is recommended to take the medication at the same time daily (e.g., in the morning). For patients with potential intravascular volume depletion (e.g., patients receiving diuretics, especially with kidney dysfunction), it is advised to monitor their condition and start treatment at a lower dose. <for elderly> No dosage adjustment is necessary when administering this medication to elderly patients aged 65 years and older. Overall differences in efficacy and safety between elderly and younger patients have not been observed, but some elderly patients may exhibit heightened sensitivity.
Child dose
Safety and efficacy in children have not been established.

Adult dose

The recommended initial dose for adults is 10 mg once daily, irrespective of meals. If blood pressure is not adequately controlled at this dose, it may be increased to 20 mg once daily. If further reduction in blood pressure is needed, it may be increased to a maximum of 40 mg once daily. Precautions

This medication may cause serious harm or death to the fetus or newborn if taken during the second and third trimesters of pregnancy; therefore, it should be discontinued immediately if pregnancy is confirmed. Individuals who are allergic to this medication or its components, those who are pregnant or breastfeeding, patients undergoing kidney dialysis, severely impaired liver patients, patients with biliary obstruction, and individuals with certain genetic lactose intolerance should not take this medication. Patients with reduced blood volume or salt levels may experience hypotension; therefore, it is important to check their condition before starting treatment. Patients with severe heart failure, whose kidney function relies on the renin-angiotensin-aldosterone system, should exercise caution as kidney function may change. Patients with poor kidney function or those who have received a kidney transplant should have their blood potassium and creatinine levels monitored regularly during treatment with this medication. Patients with renovascular hypertension are at risk of worsened kidney function and should be monitored closely. This medication can increase blood potassium levels, so caution is required for patients with heart failure or kidney dysfunction, and blood potassium levels should be checked frequently. Patients with valvular heart disease or obstructive cardiomyopathy should also be careful when taking this medication. Patients with primary aldosteronism may be unresponsive to this medication. Due to its blood-pressure-lowering effect, this medication may cause dizziness, so activities like height work and driving should be avoided. The antihypertensive effect may be somewhat less in Black patients. Common side effects include dizziness, headache, back pain, indigestion, and cough, though most are mild and transient. Rarely, symptoms such as chest pain, swelling, abdominal pain, palpitations, and rash may occur; if these persist, consult a doctor. Liver function abnormalities may rarely occur during treatment with this medication, so regular liver function tests are advised. In cases of chronic diarrhea and significant weight loss, chronic malabsorption related to this medication may be suspected; inform healthcare providers immediately. If taken with potassium-sparing diuretics or potassium supplements, the risk of dangerously high blood potassium levels increases, so caution is necessary. When used with other antihypertensives, the blood pressure-lowering effect may be enhanced, so inform healthcare providers of all medications being taken. Co-administration with lithium preparations may lead to increased lithium levels and toxicity, so they should not be used together. Co-administration with non-steroidal anti-inflammatory drugs may increase the risk of worsened kidney function, so sufficient fluid intake and kidney function monitoring are necessary. Taking this medication with other agents that act on the angiotensin system increases the risk of hypotension, hyperkalemia, and kidney dysfunction, so careful monitoring is required. Pregnant women should avoid this medication to protect the health of the fetus and newborn; if breastfeeding, they should consider discontinuing breastfeeding or the medication as adverse reactions may occur in infants. Safety in children has not been established, so it is advisable not to use this medication in pediatric patients. Elderly patients may have increased drug concentrations in their bodies, so dosage adjustment and monitoring are necessary. In the case of overdose, symptoms such as hypotension and palpitations may occur, requiring immediate treatment.
