Summary

Main use
Relief of moderate to severe acute and chronic pain
Who takes it
Adults · Children
Adult dose
Children over 12 years and adults should adjust the dosage based on the severity of pain and therapeutic response. An initial dose of 4 tablets is recommended, and subsequent dosing intervals should be no less than 6 hours, not exceeding 16 tablets per day. The lowest effective dose should be administered for the shortest duration possible, and long-term treatment should be avoided unless necessary due to the nature and severity of the illness, with regular monitoring to determine whether continued use is required. Elderly Administer the usual adult dosage; however, for those aged 75 and older, the dosing interval should be a minimum of 6 hours due to a 17% increase in the elimination half-life of tramadol when taken orally. Renal Impairment For moderate renal impairment (creatinine clearance of 10-30 ml/min), the dosing interval should be extended to 12 hours. For severe renal impairment (creatinine clearance of less than 10 ml/min), administration is not recommended. Tramadol is removed very slowly during hemodialysis and filtration, so re-dosing of this medication after dialysis is not necessary to maintain pain relief. Hepatic Impairment For moderate hepatic impairment, caution should be taken in extending the administration interval. This medication should not be administered to patients with severe hepatic impairment.
Child dose
Children over 12 years should follow the same dosage adjustment criteria as adults.

How to take

Precautions

This medication can cause liver damage, so individuals who regularly consume more than three alcoholic beverages daily should consult a physician or pharmacist before taking it. Additionally, serious skin rashes or blisters and severe allergic reactions may occur rarely; if a rash or hypersensitivity reaction appears, the medication should be discontinued immediately. One of the active ingredients, tramadol, may increase the risk of seizures if the recommended dosage is exceeded, and this risk may be higher when taken with certain antidepressants or other central nervous system medications, so caution is advised. Rarely, severe allergic reactions such as anaphylaxis can occur, which may be life-threatening in patients with acute bronchial asthma; therefore, special caution is required. Taking this medication with alcohol or central nervous system depressants can cause severe drowsiness, respiratory depression, or coma, so it is advisable to avoid combinations. In patients with increased intracranial pressure or head injuries, this medication may worsen their condition due to respiratory depression, so careful use is necessary. This medication may cause dependence similar to morphine; thus, long-term use should consider the risk of dependence and abuse. Particularly in patients with impaired liver function, this medication’s ingredient acetaminophen may lead to liver damage, so it should be taken only after consulting a physician. Moreover, patients with certain genetic metabolic characteristics may accumulate excessive amounts of the active ingredient, increasing the risk of respiratory depression, so caution is needed. This medication is not to be given to children under 12 years of age or adolescents under 18 years who have undergone tonsillectomy or adenoidectomy. Patients with sleep apnea or hypoxia may experience worsening symptoms after taking this medication, so continuous monitoring is required. Rarely, it can cause decreased sodium levels in the bloodstream, necessitating regular monitoring, especially in elderly patients or those with risk factors. In case of overdose, severe respiratory depression, coma, or seizures may occur, and emergency treatment should be sought immediately; treatment for acetaminophen overdose may be necessary to protect the liver. This medication may cause drowsiness and dizziness when driving or operating machinery, so caution is needed, and the risk of these side effects increases when taken with alcohol or other central nervous system depressants. Finally, store this medication in a safe place out of reach of children to prevent accidental ingestion or misuse.
