Treatment of gastric and duodenal ulcers and gastritis (gastrointestinal diseases)
This medication is effective for ulcers occurring in the stomach and duodenum, gastritis, and conditions associated with excessive gastric acid secretion such as Zollinger-Ellison syndrome. It is also used for the treatment of gastroesophageal reflux disease and is administered prior to anesthesia to prevent Mendelson syndrome. It is helpful in treating ulcers that may occur after surgery or ulcers caused by nonsteroidal anti-inflammatory drugs (NSAIDs). It alleviates various gastrointestinal disorders, reducing patients' discomfort.
○ Adults Typically taken without regard to meals, 2 tablets twice a day orally in the morning and before bedtime. 1) Gastric and duodenal ulcers Administer 2 tablets twice daily or 4 tablets once daily before bedtime. For duodenal ulcer patients, administering 4 tablets twice daily in the morning and before bedtime can yield higher treatment rates. Dosage remains the same for patients with NSAID-induced ulcers or in combined therapy for preventing gastric and duodenal ulcers during long-term NSAID therapy. Usually, conditions caused by NSAID use heal within 8 weeks, but healing may take longer in some patients. For patients with recurrent ulcers who had good results on short-term therapy with this medication, a maintenance dose of 2 tablets once daily before bedtime is recommended. 2) Gastroesophageal reflux disease For 8 weeks, administer 2 tablets twice daily or 4 tablets once daily at bedtime. However, for severe gastroesophageal reflux disease, the dosage may be increased to 4 tablets four times a day. 3) Zollinger-Ellison syndrome Begin treatment with an initial dose of 2 tablets three times daily and may increase as needed. In severe disease, a total of up to 6 g of ranitidine may be directed by a physician. 4) Peptic ulcers, acute stress ulcers, and upper gastrointestinal bleeding due to acute mucosal lesions Once oral administration becomes possible, switch from injection to oral administration, administering 2 tablets twice daily. 5) Preoperative administration (to prevent Mendelson syndrome) Administer 2 tablets 2 hours before anesthesia induction. In this case, it is preferable to also take 2 tablets the night before surgery.
This medication should not be used in patients receiving dialysis or those who have had allergic reactions to this drug or its components. Patients with a history of acute porphyria should also avoid this medication, and it is not safe to administer to patients with impaired kidney function or those who have recently undergone upper gastrointestinal surgery. This drug should not be administered to pregnant women or women who may become pregnant, neonates, and those on dietary restrictions limiting potassium intake. Patients with liver dysfunction, a history of drug allergies, elderly patients, or those with any kidney abnormalities should use this medication cautiously. Patients using nonsteroidal anti-inflammatory drugs should have regular checks, especially elderly patients or those with a history of peptic ulcers. Because this medication can mask symptoms of gastric cancer, it should only be administered to patients with gastric ulcers or newly appeared dyspeptic symptoms after confirming malignancy. Patients with Helicobacter pylori positive gastric and duodenal ulcers will need bacterial eradication treatment. Caution is necessary for severely ill patients, patients with hypophosphatemia, or those having difficulty swallowing when using this medication. During the use of this medication, allergic reactions may occur, including shock, skin rash, hives, and breathing difficulties; symptoms should be reported immediately and the medical personnel should be consulted. Blood tests may show decreased white blood cells or platelets; if symptoms such as fatigue, bleeding, or fever occur, tests should be conducted. Symptoms of liver dysfunction may develop, so jaundice or fatigue should be reported immediately. Cardiovascular abnormalities such as arrhythmias or vasculitis have been reported rarely, so caution is advised. Gastrointestinal symptoms such as nausea, vomiting, constipation, diarrhea, and abdominal distension may occur, and stools may turn black, but this usually does not require special treatment. In severely ill patients or the elderly, neurological abnormalities such as dizziness, drowsiness, confusion, or seizures may occur, necessitating observation. Muscle pain or joint pain may occur, and rarely severe muscle disorders like rhabdomyolysis can happen; report any unusual symptoms immediately. Skin rashes or hair loss may occur rarely. Avoid prolonged continuous use, and change to another medication if there is no therapeutic effect. In elderly patients or those with chronic conditions, there may be an increased risk of pneumonia, so caution is necessary. Avoid driving or operating dangerous machinery if experiencing dizziness or drowsiness while taking this medication. Use of this medication may interact with other drugs, so particularly when taken with anticoagulants, antibiotics, or anticonvulsants, consult with a healthcare professional. It is safest not to use this medication during pregnancy or breastfeeding. Safety in children has not been established, and its use is not advisable. This medication may affect some test results, so it is important to inform the medical staff during testing. In case of overdose, emergency procedures such as vomiting or gastric lavage may be needed, so caution is advised. This medication reduces gastric acid secretion, which could promote bacterial proliferation in the gastrointestinal tract; therefore, regular check-ups, including ocular examinations, are necessary with prolonged use.
