The infiltrative and nerve block anesthesia used in dental procedures is a method of eliminating pain by blocking the nerves in specific areas. Nerve block anesthesia blocks larger nerves to eliminate sensation over a wide area, while infiltrative anesthesia is given directly into the tissues surrounding the targeted area to induce local anesthesia. Both methods are used to reduce pain during dental treatment and provide a comfortable treatment environment.

How it is given

Adult administration

For nerve block or infiltration anesthesia, use lidocaine hydrochloride ranging from 6-40 mg (equivalent to 0.3-2 ml). In oral and maxillofacial surgery, use lidocaine hydrochloride ranging from 60-100 mg (equivalent to 3-5 ml).

Precautions

This drug should not be used in patients with severe bleeding or in shock. It should also not be administered to patients with inflammation at the injection site or surrounding area, or to patients with sepsis. Patients allergic to this drug or amide-type local anesthetics, as well as those sensitive to vasoconstrictors, should be cautioned. Precautions should be taken when used in patients with hypertension, arteriosclerosis, heart failure, hyperthyroidism, diabetes, or vascular spasms. Special caution must be exercised for patients with central nervous system diseases, pregnant women, elderly patients, and those with severe spinal deformities. Patients taking inhalational anesthetics such as halothane or certain antidepressants should also be cautious. Additionally, this drug contains sulfites, which may trigger allergic reactions or asthma attacks, therefore asthmatic patients should be especially cautious. Symptoms of shock such as decreased blood pressure, pallor, abnormal pulse, or respiratory depression may occur, requiring close observation of the patient. Malignant hyperthermia, a serious side effect, may rarely occur, in which case the drug should be discontinued immediately and proper treatment should be administered. Central nervous system symptoms may include tremors or seizures, and symptoms such as drowsiness, anxiety, dizziness, and nausea should be closely monitored. Allergic reactions such as hives or swelling may occur, so immediate notification is required in case of unusual symptoms. While using local anesthetics, it is important to be prepared for emergencies in case of sudden shock or toxic symptoms. It should be administered in minimal doses and in dilute concentrations, ensuring that the injection needle does not enter the blood vessel and that the injection rate is kept slow for safety. This drug should only be administered in pregnant women or those with potential for pregnancy if the benefit outweighs the risks. Since the components of this drug may be excreted in small amounts in breast milk, nursing mothers should also consult a physician before administration. The elderly may be more sensitive to the epinephrine contained in this drug, requiring careful monitoring and cautious administration. Particularly during epidural anesthesia, attention should be paid to the dosage adjustment as the anesthetic range may increase. Lastly, this drug should not be used for surface anesthesia in ophthalmology.
