Treatment of essential hypertension and reduction of cardiovascular disease risk
This medication is used for the treatment of essential hypertension. Additionally, it is administered to high-risk diabetic patients aged 55 and older who are intolerant to ACE inhibitors, with a history of coronary artery disease, peripheral artery disease, stroke, transient ischemic attack, or evidence of end-organ damage, to help reduce the risk of myocardial infarction, stroke, and death from cardiovascular disease. In other words, this medication is effectively used for blood pressure control and reduction of cardiovascular risk.
Telmisartan should be taken orally at a dose of 40 mg once daily with water, regardless of meals. In some cases, a dose of 20 mg once daily may be effective depending on the patient. If necessary, the dose can be increased to 80 mg once daily. For reducing the risk of cardiovascular disease, a dose of 80 mg once daily is recommended. Patients with liver impairment should not exceed a dose of 40 mg once daily.
This medication can cause serious harm to the fetus and newborn if taken during pregnancy, so treatment should be discontinued immediately upon confirmation of pregnancy. It should not be used in individuals with allergies to this medication or its ingredients, in pregnant or breastfeeding women, individuals with severe liver disease, patients with a history of certain vascular swelling conditions, and patients with diabetes or severe kidney impairment. Elderly individuals, patients with hyperkalemia, liver disease, certain heart conditions, gastrointestinal disorders, and renal artery hypertension should use this medication with caution. During treatment, various side effects may occur, including headache, dizziness, cough, dyspepsia, skin rash, muscle pain, and renal function abnormalities. If any unusual symptoms occur, immediate consultation with healthcare professionals is required. Particularly, if sudden low blood pressure, severe allergic reactions, angioedema, hyperkalemia, or renal function deterioration occurs, treatment should be stopped immediately and appropriate medical care should be obtained. It is important to administer this medication while regularly monitoring renal function and blood potassium levels, as hypotension may occur in conditions of reduced blood volume or sodium depletion. This medication may interact with other medications that enhance or inhibit blood pressure-lowering effects, so it is essential to inform healthcare professionals of any other medications being taken. Usage should be avoided during pregnancy or breastfeeding, and particularly after the second trimester, monitoring fetal renal function and skull development is recommended. Safety and efficacy have not been established for children under 18 years of age, so use in this population is discouraged. Elderly patients are at risk of stroke and other complications due to excessive blood pressure lowering, so patient status must be closely monitored during treatment. In the event of an overdose, symptoms such as hypotension, dizziness, and acute renal failure may occur, so immediate medical treatment is essential. This medication is not removed by dialysis, therefore, caution is required. Keep this medication out of the reach of children, do not change the original packaging, and it is advisable to open the packaging just before use. During treatment, caution is warranted while driving or operating hazardous machinery, as dizziness or fainting may occur.
