This medication is used to treat essential hypertension and is effective for heart failure patients (NYHA class II to IV) who cannot use ACE inhibitors. It also helps reduce the risk of death in patients with left ventricular dysfunction or reduced contractility after myocardial infarction. Thus, this drug is used not only for blood pressure management but also for heart failure improvement and preventive treatment following myocardial infarction.

How to take

Adult dose

Essential hypertension: Take once daily 80 mg orally with water during meals or between meals, preferably at the same time every day. If blood pressure control is difficult, the dose may be increased to 160 mg daily or combined with diuretics, with a maximum increase to 320 mg daily. For elderly patients (75 years and older), treatment should start at 40 mg once daily. For patients with renal impairment (creatinine clearance 10-20 mL/min), the dosing regimen should be adapted.

Child dose

Safety and efficacy have not been established for children and adolescents under 18 years of age, so it should not be administered.

Precautions

This drug can cause serious harm to the fetus and newborn when taken during pregnancy, especially in the second and third trimesters, so it should be discontinued immediately upon confirmation of pregnancy. Women who are pregnant or may become pregnant and nursing mothers should not use this medication, and it should not be administered to patients with severe liver disease or significantly impaired kidney function. Due to the strong blood pressure-lowering effect of this medication, caution should be exercised when administering it to the elderly or patients with weakened kidney function, as well as to those with hyperkalemia, and dosage adjustments should be made for patients with impaired liver function. Patients with heart failure or following myocardial infarction may experience changes in kidney function, so regular kidney function tests should be conducted while taking it. There may be a sudden drop in blood pressure during initial use, causing dizziness or fainting; therefore, it is safe to start with a low dose and gradually increase the dose, especially for those on diuretics or who are dehydrated. This medication can increase blood potassium levels, so potassium supplements or potassium-sparing diuretics should be monitored carefully for blood potassium concentration. Concomitant use with non-steroidal anti-inflammatory drugs (NSAIDs) can reduce blood pressure-lowering effects and worsen kidney function; caution is required when using these together. When used with lithium preparations, lithium levels may rise and cause toxicity, so blood lithium concentrations should be frequently checked when used together. This medication contains Yellow No. 5 dye, so patients allergic to this component should use it cautiously. Due to symptoms like dizziness or drowsiness, caution should be taken while driving or operating machinery. In case of overdose, symptoms such as low blood pressure or decreased consciousness may occur, and immediate medical attention is required. The safety and effectiveness of this drug have not been established in children and adolescents, so it is advisable not to use it in this age group. It is important to regularly monitor blood pressure, kidney function, and electrolyte levels to ensure there are no abnormalities during treatment with this medication.
