Adequate supplementation of selenium for patients receiving total parenteral nutrition (TPN) is crucial to prevent selenium deficiency and enhance immune function and antioxidant action. Selenium protects against cell damage and regulates inflammatory responses, thus sufficient selenium supply during TPN can assist in the recovery and health maintenance of patients. Therefore, selenium supplementation during parenteral nutrition is used to improve nutritional imbalance and prevent complications.

Dosage and Administration

Adult dosage

During stable metabolism in total parenteral nutrition (TPN), administer 20 to 40 μg of selenium daily (this medication contains 40 μg/mL of selenium).

Children's dosage

Administer 3 μg per kg of body weight per day. Monitor plasma selenium levels frequently to reference for the next dosage.

Precautions

This medication contains selenium and should not be used in patients with selenium poisoning. Toxicity may occur with overdose, so use should be discontinued immediately if abnormal symptoms arise. Injecting this medication directly into the peripheral vein without dilution can cause phlebitis, so it must be diluted before administration. This medication is a stable solution, and therefore must be mixed before administration. Patients with poor kidney function or premature infants should use this medication with caution, and attention is needed due to the risk of aluminum toxicity with long-term administration. Rarely, pain or allergic reactions may occur during injection; if such symptoms appear, discontinue use and consult a physician. If symptoms such as dermatitis, foul smell during sweating or breathing, hair loss, restlessness, vomiting occur, discontinue use immediately and consult a specialist. Before administration, ensure there are no precipitates or discoloration, and it must be prepared under aseptic conditions. Patients with weak kidney or gastrointestinal function may have difficulty excreting selenium, which may require dose adjustments, and selenium levels should also be periodically monitored in transfusion patients. The safety of this medication has not been established in pregnant or nursing women, so it should only be used when the benefits of treatment outweigh the risks in cases of pregnancy possibility or in pregnant women. During breastfeeding, it is advisable to discontinue either the medication or breastfeeding based on the importance of drug administration. Overdose may cause abnormalities in nail and hair growth, dermatitis, oral abnormalities, gastrointestinal issues, tension, depression, a metallic taste, vomiting, and a garlic odor from the mouth. In severe cases, life-threatening symptoms such as vascular collapse or pulmonary hemorrhage may occur, requiring immediate emergency treatment. In cases of overdose, gastric lavage or diuresis, high-dose vitamin C may be needed, and in extreme cases, dialysis may be necessary to remove selenium. Keep out of reach of children, store in a dry place at room temperature, and use immediately after opening. Transferring to other containers may cause quality degradation or accidents, so caution is necessary.
