Supplementation effect for acute and chronic renal failure (amino acid loss after peritoneal dialysis or hemodialysis).
Amino acids can be lost in patients with acute and chronic renal failure after peritoneal dialysis or hemodialysis. In such cases, it is necessary to properly supplement amino acids. This improves the nutritional status of the body and aids in the recovery of the patient. The supplementation of amino acids prevents nutritional deficiencies after dialysis and plays an important role in maintaining overall health. Therefore, it effectively manages the nutritional status of patients undergoing dialysis treatment for acute and chronic renal failure.
The total amino acid dosage is 0.5-1.5g per day per kg of body weight, administered through intravenous drip infusion at maximum capacity. The infusion rate should ideally be 20 drops over 1 minute. Calories should be administered orally or non-orally before or concurrently with this medication.
In children, the dosage and infusion rate should be adjusted accordingly.
This medication must be used according to a doctor's instructions. It should not be used in patients with hepatic coma or at risk for hepatic coma, patients with severe electrolyte imbalances, patients with reduced circulatory blood volume, patients with hyperammonemia, patients with congenital amino acid metabolism disorders, patients with heart failure, patients with overhydration, and patients with a history of myocardial infarction. It should be administered with caution in patients with congestive heart failure, children, low-weight patients, patients dependent on this medication, and those with complications such as gastrointestinal bleeding. If systemic itching, rash, or urticaria-type hypersensitivity reactions occur while using this medication, it should be discontinued. Gastrointestinal symptoms such as nausea, vomiting, and loss of appetite, as well as circulatory symptoms such as chest discomfort and palpitations, may also occur. Rapid and large administration may lead to acidosis, so caution is required. Liver function test values may increase, and blood urea nitrogen (BUN) levels may rise, while chills, fever, headache, and vascular pain may occur rarely. When administered via peripheral veins, there is a risk of thrombophlebitis. This medication is used for patients requiring strict low-protein diets, those with acute or deteriorating conditions, dialysis patients, or patients unable to or deficient in oral nutrition. If amino acid supply is primarily dependent on this medication, symptoms of hyperammonemia or altered consciousness may arise, necessitating discontinuation if symptoms occur. Fluid, electrolyte, acid-base status, and BUN levels should be regularly monitored and managed. Especially for children, patients with liver dysfunction, and those with high caloric needs, specially formulated solutions should be used. Diabetic patients or uremic patients may require insulin for blood sugar management when administering high concentrations of glucose. Administering amino acids alone may lead to ketone accumulation, so it is recommended to also provide carbohydrates. In chronic renal failure patients and acute renal failure patients, the intake of protein and calories in meals should be carefully adjusted during administration. Mixing this medication with other drugs may produce toxic substances, so only drugs confirmed to be compatible should be used immediately under strict aseptic conditions. In the case of overdose, if edema or intolerance symptoms occur, it is necessary to discontinue or reduce the dose and administer it slowly. Since electrolytes are included, care should be taken regarding electrolyte balance when administered in large quantities or with electrolyte solutions. When administering, it should be slowly infused into the vein, as rapid infusion may cause chills and vomiting and lead to amino acid loss. During cold conditions, the drug should be warmed to body temperature for use. Rigorous aseptic conditions must be maintained during the preparation and management of the infusion, with particular care taken to sterilize injection devices. If the solution is cloudy or discolored, it should not be used, and it should be used promptly after mixing, limiting refrigeration to within 24 hours. If crystals appear, heat to 50-60 degrees Celsius to dissolve and cool to body temperature for use. Store the drug solution in a sealed container until opened, and do not reuse the remaining solution after opening.
