This drug is used to enhance blood sugar control in patients with type 2 diabetes, in conjunction with dietary therapy and exercise therapy. It can be administered alone, and in cases where blood sugar control is difficult with oral hypoglycemic agents, it may be used in combination with insulin. It may also be used together with metformin in cases where blood sugar control is inadequate with either sulfonylurea or metformin monotherapy. This effectively improves blood sugar control, aiding in diabetes management.

How to take

The drug is taken orally once a day, before breakfast or immediately before the first meal. For patients who have not received medications before, treatment starts with 1 mg of glimepiride, increasing 1 mg at 1 to 2 week intervals. Generally, additional effects are minimal at doses above 4 mg, but may be increased to 6 mg or 8 mg if necessary. If 4 mg daily is insufficient, switching to insulin therapy should be considered.

Precautions

This drug should not be administered to patients with insulin-dependent diabetes, diabetic ketoacidosis, or those in a coma. Patients who have had allergic reactions to this drug or to sulfonylureas should also not use it. It is advisable not to use this drug in patients with severely impaired liver or kidney function, and switching to insulin therapy is recommended in such cases. Women who are pregnant, may become pregnant, or are breastfeeding should not take this drug. Insulin therapy is more appropriate in patients with severe infections or before and after surgery or in cases of severe trauma. Close monitoring is necessary, especially at the beginning of treatment, as the risk of hypoglycemia is particularly high in elderly patients, poorly nourished patients, or patients with irregular eating patterns. Patients frequently consuming alcohol or with reduced kidney and liver function are also at increased risk of hypoglycemia. Caution is needed for patients who overdose on this drug or have endocrine disorders, as well as those taking other medications. Symptoms of hypoglycemia may include headache, severe hunger, nausea, fatigue, confusion, drowsiness, and loss of consciousness; upon experiencing symptoms, immediate intake of sugar is essential. Rare adverse effects may include visual disturbances, gastrointestinal symptoms, liver dysfunction, blood disorders, and allergic reactions; if any unusual symptoms occur, immediate consultation with a doctor is required. To control blood sugar levels, it is important to practice dietary management and exercise therapy in addition to taking this drug. When experiencing hypoglycemia, patients should always carry sugar-sweetened beverages or sugar, as artificial sweeteners are ineffective. Particularly in stressful situations, surgeries, or infections, blood sugar control may become challenging, and switching to insulin therapy may be necessary. The dosage for elderly patients with decreased renal function should be adjusted accordingly. In cases of overdose, appropriate emergency measures such as inducing vomiting, preventing drug absorption, or administering glucose injection are required, and long-term monitoring is essential since there is a potential for recurrent hypoglycemia. Keep this medication out of reach of children and store it in its original container to prevent misuse or quality degradation. This drug is not recommended for pregnant or breastfeeding women; if necessary, switching to insulin therapy is advisable.
