This medication is used for hypoproteinemia, malnutrition, and amino acid supplementation before and after surgery.
Dosage and Administration
Adult dosage
As a total amino acid dosage, administer 1-1.5 g per kg of body weight per day as a maximum intravenous infusion. The infusion rate should be approximately 10 g of total amino acids over 60 minutes, with 30-40 drops to be infused over 1 minute as a principle.
Children's dosage
In pediatric patients, dosage and infusion rates should be adjusted appropriately.
Precautions
This drug must be used strictly according to a physician's instructions. It should not be used in patients with hepatic coma or at risk of hepatic coma, patients with severe kidney dysfunction, patients with nitrogen metabolism disorders, patients with severe heart failure, patients with pulmonary edema, patients with very low or no urine output, and patients with hypernatremia or alkalosis due to sodium or chloride ions. Cautious administration is required in patients with severe acidosis, congestive heart failure, patients with edema due to sodium retention, hyperkalemia or potassium retention, and patients with impaired liver or kidney function. This drug contains sodium bisulfite, which may cause allergic reactions in patients with sulfite allergy, particularly asthmatics, thus caution should be taken. Rarely, hypersensitivity reactions such as skin rash may occur, and in such cases, use should be discontinued immediately. Gastrointestinal symptoms such as nausea or vomiting, discomfort in the chest, palpitations, rapid heartbeat, and elevated blood pressure may occur. Rapid large-volume administration may lead to metabolic acidosis, requiring caution. Chills, fever, headache, dyspnea, respiratory arrest, shock, bronchospasm, urinary cramps, and other symptoms may occur. Metabolic complications such as metabolic acidosis, hypophosphatemia, alkalosis, hyperglycemia, diabetes, dehydration, hypoglycemia, elevated liver enzymes, vitamin imbalance, and electrolyte imbalance, or hyperammonemia in children may develop. Patients with impaired liver or kidney function may show elevated blood urea nitrogen (BUN) levels, and patients with liver dysfunction may exhibit severe symptoms such as serum amino acid imbalance, metabolic alkalosis, and coma. When administered in large doses, electrolyte imbalances may occur, thus caution is needed when administering with electrolyte-containing solutions to maintain electrolyte balance. Excessive doses may lead to metabolic alkalosis, hence solutions containing acetate ions should be used cautiously. Patients with kidney disease, lung disease, or severe heart disease should be monitored closely to avoid excessive fluid retention during administration. Patients with diabetes should frequently monitor their blood sugar levels while on this medication. The protein-sparing effect may be reduced when used together with tetracycline antibiotics. The safety of this medication in pregnant women has not been adequately established, thus it should only be administered when the therapeutic benefits outweigh the risks. In pediatric patients, the administering concentration should be kept low due to immature liver function, and blood ammonia levels should be frequently tested. Elderly patients may have reduced physiological functions, so the administration rate should be slow and the dosage adjusted accordingly. This medication contains electrolytes; therefore, caution is necessary regarding electrolyte balance during large-volume administration, and if the infusion rate is too fast, side effects such as chills or vomiting may occur. Before administration, it is advisable to warm the solution to body temperature and take special care in maintaining the sterility of the syringe apparatus. Do not use the solution if it is cloudy, discolored, or leaking, and dispose of any remaining solution within 24 hours after use. During storage, it should be kept sealed to prevent exposure to air or moisture. It is important to maintain the storage temperature between 1 and 30 degrees Celsius to ensure the safety and efficacy of the medication.
