For patients with essential hypertension, this refers to cases where blood pressure is not adequately controlled despite the use of a combination therapy with telmisartan and amlodipine. In this case, it may be necessary to reassess the treatment plan and consider additional medication adjustments or lifestyle changes. Continuous observation and management are crucial when blood pressure management for essential hypertension is not smooth, and we aim to find the optimal treatment for the patient’s condition to assist in blood pressure control.

How to take

Adult dose

This medication is taken once daily, 1 tablet, with or without food. It is recommended to take it at the same time every day (e.g., in the morning). For patients whose blood pressure is not controlled with the combination therapy of telmisartan/amodipine, this drug 40/5/12.5 mg may be administered, and the dose may be adjusted considering the patient's blood pressure response with intervals of at least 2 weeks. The maximum recommended dose of this drug is 80/5/25.

Precautions

This medication can cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters, so women who are pregnant or may become pregnant should not take it. Patients with impaired liver function or those with ongoing liver disease should exercise caution, as this drug may induce hepatic coma. Patients with severely impaired kidney function may experience increased blood nitrogen levels due to the diuretic component of this drug, so caution is required upon administration. Patients with allergies or a history of hypersensitivity to this drug or its ingredients should not take it. Patients with obstructive biliary disease, severe liver impairment, severe aortic valve stenosis, or in a state of shock should also avoid this medication. For patients with diabetes and moderate to severe renal impairment, concomitant use of this medication with certain drugs should be avoided. Women who are breastfeeding should not take this medication, and if currently taking it, it is recommended to discontinue breastfeeding. Symptoms such as low blood pressure, dizziness, headaches, and swelling may occur during treatment; therefore, if symptoms are severe, consultation with a physician is advised. Regular blood tests should be performed to monitor for electrolyte imbalances in the blood, particularly with attention to potassium and sodium levels. When administering to elderly patients, it is essential to start with a low dose and gradually increase it, while being careful to prevent blood pressure from dropping too low. In case of overdose, symptoms such as low blood pressure, dizziness, and irregular heartbeat may occur; immediate contact with a healthcare institution and appropriate treatment should be sought. This drug may interact with other medications, so it is essential to inform a physician or pharmacist of any other medications being taken. Caution is necessary when driving or operating hazardous machinery, as drowsiness or dizziness may occur. This drug is not recommended for children as its safety and efficacy have not been established.
