How to take

Adult dose

This medication should be taken once daily with the evening meal. The 5/1000 mg extended-release tablets should be taken once daily, 1 tablet at a time. Patients taking the 2.5/1000 mg extended-release tablets should take 2 tablets at the same time once daily. The initial dosage consists of a total daily amount of linagliptin 5 mg and metformin hydrochloride 1000 mg, with metformin hydrochloride having a maximum recommended daily dosage of 2000.

Precautions

This medication contains components that help control blood sugar, namely linagliptin and metformin hydrochloride. This medication may rarely cause a dangerous condition called lactic acidosis; therefore, it should not be used in patients with impaired kidney function, liver function damage, dehydration, severe infections, or acute conditions such as acute myocardial infarction. Also, administration should be avoided in patients with type 1 diabetes, diabetic ketoacidosis, acute alcohol intoxication, chronic alcoholism, or severely malnourished patients. During treatment with this medication, hypoglycemia may occur, so blood glucose changes should be closely monitored, especially if used concurrently with insulin or sulfonylureas. Symptoms of lactic acidosis include muscle cramps, abdominal pain, and weakness; if any of these symptoms occur, the patient should immediately inform their doctor. Regular kidney function tests should be performed both before and during treatment to monitor renal status; this medication is considered unsafe if kidney function declines. Patients with weakened heart function or congestive heart failure need to be cautious when using this medication. If iodinated contrast is to be used for radiographic studies, this medication should be temporarily discontinued for a period before and after the examination, and should also be stopped for a period before and after surgery. Acute pancreatitis or skin blistering resembling bullous pemphigoid can occur during treatment; thus, if abdominal pain or skin abnormalities occur, the patient should consult healthcare professionals immediately. The metformin component may interfere with vitamin B12 absorption, so regular monitoring of vitamin B12 levels is advisable for long-term use. This medication should not be crushed or chewed, and should be swallowed whole; it is normal for the tablet shell to be excreted. There might be interactions with other medications, so if the patient is taking other drugs, it is essential to inform their doctor or pharmacist. The use of this medication is generally not recommended during pregnancy or breastfeeding, and safety has not been established for children. Elderly patients frequently experience decreased kidney function, so careful monitoring of renal function is necessary when using this medication. In case of overdose, gastrointestinal disturbances or lactic acidosis may occur, so it is important to adhere to the prescribed dosage and seek medical attention immediately if unusual symptoms arise. Finally, this medication should be kept out of the reach of children, and stored in a cool, dry place away from direct sunlight and moisture.
