How to take

Adult dose

This medication should be taken once daily with meals, preferably with the evening meal. The dosage should be determined based on each patient's current treatment regimen, efficacy, and tolerability, not exceeding Linagliptin 5 mg and sustained-release Metformin Hydrochloride 2000 mg. The 5/1000 mg sustained-release tablet should be taken once daily as 1 tablet, whereas the 2.5/1000 mg sustained-release tablet should be taken once daily as 2 tablets simultaneously. 1)

Precautions

This medication contains two components, Metformin and Linagliptin. Metformin can rarely cause a dangerous condition known as lactic acidosis, which can lead to death. Therefore, patients at high risk for lactic acidosis should not take this medication. For example, patients with severely impaired kidney function, those with poor liver function, patients suffering from acute infections or dehydration, and those with severe heart problems should avoid this medication. Additionally, patients allergic to the components of this medication or those with type 1 diabetes and diabetic ketoacidosis should also refrain from use. When taking this medication, caution must be exercised regarding the risk of hypoglycemia. Particularly when used in combination with other diabetes medications, such as sulfonylureas or insulin, the risk of hypoglycemia may increase, so the dosage should be adjusted according to a doctor's guidance. Symptoms of lactic acidosis may include muscle cramps, abdominal pain, and severe fatigue, so patients should immediately inform healthcare providers if such symptoms occur. Moreover, this medication should be temporarily discontinued in situations where acute deterioration of kidney function may occur or if kidney function is deteriorating. Regular kidney function tests should be done before and during treatment, especially in elderly patients or those with slightly reduced kidney function. Patients with unstable heart function or severe heart failure should use this medication with caution, and it should be temporarily discontinued before and after the administration of iodinated contrast during radiologic investigations. Skin conditions such as acute pancreatitis or similar pemphigus may occur, so patients should consult a doctor immediately if they experience severe abdominal pain or skin abnormalities. This medication should be swallowed whole without breaking, chewing, or crushing, and the tablet shell may be expelled in stool, but this is normal. Metformin can lower vitamin B12 levels, so it's advisable to monitor vitamin B12 levels through blood tests with long-term use. This medication may interact with other drugs; therefore, it is essential to inform healthcare providers about any other medications being taken, especially those that can affect kidney function. It is recommended that women who are pregnant or breastfeeding avoid this medication, and if planning to become pregnant or during pregnancy, consult a doctor to determine the appropriate treatment method. The safety and efficacy in pediatric patients have not been established, and elderly patients should adjust the dosage according to kidney function and monitor kidney function regularly. In cases of overdose, there is a risk of lactic acidosis; therefore, appropriate treatment should be administered immediately in a hospital, and blood dialysis may be necessary. Finally, this medication should be stored out of the reach of children, kept away from moisture, and not transferred to another container.
