View approved original text

How to take

Adult dose

This medication should be taken once daily with the evening meal. The 5/1000 mg extended-release tablet is taken once daily as 1 tablet, while the 2.5/1000 mg extended-release tablet is taken as 2 tablets at once once daily. The initial dose starts with Linagliptin 5 mg and Metformin Hydrochloride 1000 mg, and Metformin Hydrochloride can be gradually increased up to a maximum recommended daily dose of 2000 mg.

Precautions

This medication helps control blood sugar levels. However, it can rarely cause a dangerous condition known as lactic acidosis, so caution is required. Lactic acidosis occurs when there is an excessive buildup of lactic acid in the body and can be life-threatening if not treated. This medication should not be used in individuals with an allergy to it, type 1 diabetes, severely impaired renal function, or acute infections or serious illnesses. Additionally, it should not be administered to patients with liver dysfunction or those who are alcohol dependent. When using this medication, hypoglycemia may occur, so blood glucose levels should be monitored frequently, especially when taken together with insulin or sulfonylureas. Patients with diminished renal function should have regular kidney function tests, and if kidney function falls below a certain threshold, this medication should not be taken. Caution is also advised for patients with compromised heart function. If iodinated contrast agents are used during radiological examinations, this medication should be discontinued before and after the examinations and renal function should be checked. Patients scheduled for surgery should stop taking this medication 48 hours before the surgery and can resume it once kidney function has been confirmed to have recovered afterward. If sudden abdominal pain, vomiting, severe joint pain, skin rash, or blisters occur during the use of this medication, consult a physician immediately. Long-term use may lead to vitamin B12 deficiency, so regular blood tests to check vitamin B12 levels are advisable. This medication should be swallowed whole without chewing or crushing, although the tablet coating may appear in the stool, which is normal. It is advisable to avoid using this medication during pregnancy and breastfeeding, and consult a physician if planning to become pregnant. Safety in children has not been established, and elderly patients should adjust the dosage considering renal function and have regular kidney function tests. In case of overdose, the medication may not be fully absorbed in the gastrointestinal tract, leading to a risk of lactic acidosis, so immediate medical attention should be sought. This medication should be stored away from moisture and kept out of reach of children; it is not recommended to transfer it to another container.
