Precautions

This medication contains sitagliptin and pioglitazone, each with specific cautions to be aware of. First, the sitagliptin component may rarely cause serious allergic reactions. Symptoms such as severe skin swelling, rash, difficulty breathing, and anaphylaxis can occur, so if any of these symptoms appear, the medication should be stopped immediately, and a doctor should be consulted. Sitagliptin can also cause acute pancreatitis, an inflammation of the pancreas. Symptoms such as abdominal pain, nausea, and vomiting should prompt immediate medical attention. The pioglitazone component may strain the heart, so patients with existing heart failure or a history of heart failure should not take this medication. During treatment, symptoms of heart failure such as weight gain, swelling, and dyspnea may occur; if these symptoms arise, medical personnel should be notified immediately. This medication should not be given to patients with a history of bladder cancer or current bladder cancer. It should also be avoided by patients with severely impaired kidney or liver function. Women who are pregnant or may become pregnant should not take this medication. Pre-menopausal women should use appropriate contraception since this medication may promote ovulation. Patients should be vigilant for symptoms of hypoglycemia while using this medication, especially when combined with insulin or sulfonylureas, as the risk of hypoglycemia may increase. While taking this medication, if symptoms such as swelling, weight gain, joint pain, skin rash, or blisters occur, medical personnel should be informed immediately. If symptoms of liver dysfunction (fatigue, jaundice, loss of appetite, etc.) present, prompt testing is recommended. Additionally, this medication contains lactose and should not be prescribed for patients with lactose intolerance. Lastly, this medication may interact with other drugs, so if patients are taking any other medications, they must inform their doctor or pharmacist, and regular assessments of kidney and liver function are recommended.
