Dosage and Administration

Adult dosage

This medication may be administered to patients with type 2 diabetes using sitagliptin 100 mg and pioglitazone 15 mg or 30 mg, at a dose of 1 tablet once daily, regardless of meals.

Precautions

This medication contains two active components, sitagliptin and pioglitazone. First, sitagliptin helps to control blood sugar but can rarely cause severe allergic reactions. Such reactions may include anaphylaxis, angioedema, and severe skin disorders. Symptoms may manifest within the first three months of starting the medication, so caution is necessary. Additionally, sitagliptin may induce a condition known as pancreatitis, which causes inflammation of the pancreas; if symptoms like abdominal pain or nausea occur during use, patients should consult their doctor immediately. Second, pioglitazone enhances insulin sensitivity and aids in lowering blood sugar. However, this compound may worsen or newly induce heart failure, so it should not be used in patients with heart failure. During treatment, symptoms of heart failure such as weight gain, edema, and difficulty breathing should be closely monitored. Pioglitazone can cause fluid retention, leading to edema, which may place stress on the cardiovascular system; thus, patients with edema or existing heart disease should be particularly careful. Moreover, pioglitazone may increase the risk of bladder cancer; therefore, patients with a history of bladder cancer or currently diagnosed with bladder cancer should not take this medication. Dose adjustments are required for patients with impaired kidney function, and it is not to be administered to patients with severe kidney dysfunction. Caution is also necessary for patients with liver impairment. Women who are pregnant or may become pregnant should not take this medication, and premenopausal women should use appropriate contraception as this drug may stimulate ovulation. Patients should be vigilant for symptoms of hypoglycemia while taking this medication, especially if co-administered with insulin or sulfonylureas, as the risk of hypoglycemia may increase. This medication rarely causes side effects such as joint pain, skin rash, headache, dyspepsia, weight gain, edema, and others. If any adverse effects occur during treatment, patients should consult their doctor or pharmacist immediately, especially if symptoms such as an allergic reaction, signs of pancreatitis, heart failure symptoms, or bladder-related symptoms arise, and treatment should be discontinued and medical consultation sought immediately.
