This medication is used to help improve blood sugar control in patients with type 2 diabetes. Initially, it can be given alone along with diet and exercise therapy. If there is no prior experience of diabetes treatment and blood sugar control is difficult with monotherapy, it can be taken in conjunction with metformin. Additionally, it is added for combination therapy when blood sugar control is difficult with monotherapy of sulfonylureas, metformin, thiazolidinediones, or insulin. When multiple drugs are used in combination, this medication helps enhance blood sugar control. Therefore, it is effectively used to improve blood sugar control in type 2 diabetes.

Dosage and Administration

Adult dosage

This medication is administered once daily at a dose of 100 mg for monotherapy or combination therapy, with a maximum daily dosage of 100 mg. It can be taken regardless of meals. Dosage adjustment is necessary depending on renal function. - For patients with mild renal impairment (eGFR 60–90 mL/min/1.73 m2), dosage adjustment is not necessary. - For patients with moderate renal impairment (eGFR 45–60 mL/min/1.73 m2), dosage adjustment is necessary.

Precautions

This medication may cause severe allergic reactions after administration; therefore, if symptoms such as skin rash or difficulty breathing occur, it should be discontinued immediately and a physician should be consulted. Furthermore, during the use of this medication, serious pancreatitis may develop, so if symptoms such as abdominal pain or vomiting occur, it should be reported to medical personnel immediately. This medication should not be used in patients with type 1 diabetes or diabetic ketoacidosis, and should also be avoided in cases of hypersensitivity to this medication or its ingredients. Patients with impaired renal function may have difficulty excreting this medication; thus, the dosage should be carefully adjusted. This medication can cause hypoglycemia when used alone or in combination with other diabetes medications, so caution must be taken to monitor for hypoglycemic symptoms when taken with insulin or sulfonylureas. Severe joint pain may occur during medication use; therefore, if such symptoms develop, a physician should be consulted to determine whether to discontinue the medication. A rare skin disease called bullous pemphigoid has been reported; if blisters or erosions develop on the skin, a physician should be notified immediately. It is not recommended for use in pregnant or nursing women, and safety in children under 18 years of age has not been established, so use should be approached with caution. Elderly patients may experience reduced renal function; therefore, dosage adjustment and renal function tests are necessary. In cases of overdose, general supportive treatment is required; this medication can be partially removed by hemodialysis, so patients should receive treatment according to medical personnel's instructions. Lastly, interactions with other medications are generally rare, but when taken with certain medications, it is important to inform medical personnel and adjust medications if needed.
