Summary

Main use
Treatment of obesity (weight loss and maintenance); reduction and prevention of the risk of weight regain
Who takes it
Adults · Children
Adult dose
○ Adults The recommended dose of this medicine is 120 mg of orlistat orally 3 times daily, taken with a fat-containing meal or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly hypocaloric diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrates, and protein should be distributed evenly among three meals. This medicine may be omitted if a meal is missed or contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutrition. The vitamin supplement should be taken at least 2 hours before this medicine or at least 2 hours after it, such as at bedtime. Taking more than 120 mg of this medicine 3 times daily does not increase its effectiveness. Based on measurements of fecal fat, the effect of this medicine appears 24 ∼ 48 hours after administration. After treatment is discontinued, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine for periods exceeding 4 years have not been evaluated. ○ Studies have not been conducted in patients with hepatic and/or renal impairment or in older adults.
Child dose
Children under 12 years of age: Studies have not been conducted.

Precautions

This medication should not be administered to patients with chronic malabsorption syndrome or those with biliary obstruction. Patients who are allergic to this medication or any of its components should also refrain from using it. Pregnant or potentially pregnant women, as well as nursing mothers, should not take this medication. Since this medication contains lactose, patients with lactose intolerance or specific inherited metabolic disorders should not take it. Patients with liver damage or impaired liver function should take this medication with caution, and must inform their doctor immediately if symptoms of liver dysfunction occur. Patients with hyperoxaluria or calcium oxalate kidney stones should exercise caution when taking this medication. This medication can primarily cause gastrointestinal adverse reactions, and symptoms may include abdominal pain, constipation, fatty stools, and fecal incontinence. Adverse reactions are usually mild and transient, and most occur within three months, but in some cases may persist for more than six months. Due to gastrointestinal adverse reactions, some patients may discontinue the medication, so it is recommended to consult a doctor if symptoms are severe. Gastrointestinal adverse reactions may occur more frequently when this medication is taken with high-fat meals, thus it is important to balance fat intake. This medication may interfere with the absorption of fat-soluble vitamins, so taking a multivitamin containing vitamins A, D, E, K, and beta-carotene is recommended. Vitamin supplements should be taken at least 2 hours before or after taking this medication for them to be effective. Liver dysfunction symptoms include decreased appetite, itching, jaundice, dark urine, pale stools, and right upper abdominal pain, and if any of these symptoms occur, it is crucial to inform a doctor immediately. Hemostasis indicators should be regularly monitored during the use of this medication, especially for patients on oral anticoagulants. This medication may interact with certain drugs (e.g., cyclosporine, antiepileptics, antiretrovirals), so consultation with a physician is advised when co-administering. In case of overdose, observation for 24 hours is necessary, and adverse reactions usually recover quickly. Safety in children or elderly patients has not been sufficiently established, so caution is advised for use. This medication must be kept out of reach of children and should be stored in the original package away from light and moisture.
