The dosage for this medication is determined considering the current treatment regimen, efficacy, and tolerability of each patient, with sitagliptin not exceeding 100 mg and metformin not exceeding 2000 mg. This medication should be taken once daily in the evening with a meal, and the dose can be increased gradually. This medication contains sitagliptin and metformin and is used for the treatment of diabetes. Firstly, sitagliptin can cause rare but serious hypersensitivity reactions, so if an allergic reaction is suspected, discontinuation should occur immediately and a consultation with a physician is advised. Furthermore, acute pancreatitis may occur, thus careful observation is required if symptoms such as abdominal pain or vomiting arise, and discontinuation should happen if necessary. Metformin can cause a serious side effect known as lactic acidosis, which carries a higher risk in specific situations such as impaired kidney function, liver dysfunction, severe dehydration, or serious infections. Thus, patients with poor kidney function or acute illnesses should avoid using this medication. This medication is not to be administered to those with moderate or severe renal impairment, acute heart failure, acute myocardial infarction, sepsis, pregnant or breastfeeding women, as well as type 1 diabetes patients. When taking this medication, awareness of symptoms of hypoglycemia is critical, and particularly when prescribed alongside other hypoglycemic agents or insulin, the risk of hypoglycemia may increase necessitating frequent blood sugar checks. This medication is not recommended for patients with impaired liver function, and regular blood tests to monitor kidney function should be conducted for any abnormalities. Additionally, since alcohol consumption can elevate the risk of lactic acidosis, excessive drinking should be avoided during the course of this medication. If undergoing surgery or tests using contrast agents, this medication should be discontinued for a certain period, and reinitiated only after confirming normal kidney function. Elderly patients are at higher risk of reduced kidney function and may require dose adjustments and regular testing. During the use of this medication, it is essential to maintain a consistent dietary regimen and exercise, and to immediately consult a healthcare professional in case of unusual symptoms.
