  • Type 2 diabetes
  • difficulty in blood glucose control
View approved original text

This medication is used to help manage blood glucose levels in patients with type 2 diabetes. It may be administered to individuals who find it difficult to control their blood sugar levels with monotherapy when starting diabetes medications. Additionally, it serves as an alternative when monotherapy with either metformin or sitagliptin is insufficient, or when both medications are used together. It is also utilized in conjunction with metformin if blood glucose control is still inadequate when used with sulfonylureas or with thiazolidinediones. Insulin can also be supplemented with this medication if blood sugar levels are not adequately controlled. In summary, this medication is used to support blood glucose control in various situations where existing treatments are ineffective.

How to take

Adult dose

This medication is generally taken twice a day with food, and doses are gradually increased to reduce gastrointestinal side effects caused by metformin. 1. Patients not treated with metformin: Start with a dose of 1 tablet of 50/500 mg twice a day; the dose may be increased to 1 tablet of 50/1000 mg twice a day. 2. Patients being treated with metformin: Starting dose is sitagliptin 50 mg twice a day (total daily amount of 100 mg) and the existing dosage of metformin.

Precautions

This medication contains sitagliptin and metformin, which are used for diabetes treatment. First, serious allergic reactions may occur while taking this medication; if symptoms such as rash, hives, difficulty breathing, or swelling of the face or neck arise, discontinue use immediately and consult a physician. Additionally, this medication may cause pancreatitis, so if symptoms such as abdominal pain, vomiting, or nausea appear, it is important to visit a hospital promptly. Due to the metformin component, lactic acidosis—a rare yet fatal metabolic disturbance—may occur; therefore, one should be cautious of symptoms such as shortness of breath, muscle pain, fatigue, abdominal pain, and confusion. Patients with poor kidney function, severe infections, dehydration, acute heart problems, or heart failure should not take this medication. Temporary discontinuation of this medication is required before and after tests using iodinated contrast agents, and it can be resumed after kidney function is confirmed to be normal. It is safer for women who are pregnant, potentially pregnant, or breastfeeding not to take this medication. During treatment, irregular meals, excessive exercise, and interactions with other medications can increase the risk of hypoglycemia or lactic acidosis. This medication may cause side effects such as diarrhea, nausea, vomiting, abdominal pain, headache, upper respiratory infection, etc., and especially when taken with other hypoglycemic agents, symptoms of hypoglycemia may occur. Regular blood tests and kidney function assessments are necessary, and checking vitamin B12 levels is also recommended. Alcohol consumption can increase the risk of lactic acidosis; therefore, excessive drinking should be avoided during treatment. If a surgery is planned or in cases of severe infection or severe trauma, temporary discontinuation of this medication is warranted. Care should be taken during activities such as driving or operating dangerous machinery while using this medication, especially regarding symptoms of hypoglycemia or lactic acidosis. In case of overdose, seek immediate medical attention for appropriate treatment, and if necessary, the drug can be removed via dialysis.
