Treatment of essential hypertension and angina
This medication is effectively used for angina and mild to moderate essential hypertension.
1. Adults: The initial dose is 180㎎ of diltiazem hydrochloride orally once daily. The dose may be increased at 2-4 week intervals according to symptoms. The usual maintenance dose is 240-360mg once daily. The maximum daily dose is 360㎎. 2. Elderly patients and patients with hepatic or renal impairment: The initial dose is 120㎎ once daily. Measure heart rate during treatment; do not increase the dose if it falls to 50 beats or fewer.
This medication should not be used in patients with very weak heart function or problems with cardiac electrical signal transmission. Also, it should not be administered to patients with very low blood pressure or in a state of shock, pregnant women or women who may become pregnant. Patients with allergies to this medication, acute myocardial infarction, or pulmonary congestion should not use it. Caution is required when administering to patients with congestive heart failure or severe impairment of liver and kidney function as the drug's effects may be intensified. Patients with slight abnormalities in cardiac electrical signal transmission or conditions such as chronic diarrhea, which may impair proper drug absorption, should also be administered with care. The risk of side effects increases when used in conjunction with other drugs that affect the heart, such as beta-blockers. During treatment, side effects such as peripheral edema, dizziness, vasodilation, pharyngitis, diarrhea, and palpitations may occur. Various symptoms including headache, weakness, dyspepsia, dyspnea, and rash may also arise; thus, it is important to consult a doctor if any unusual symptoms appear. It is crucial to regularly monitor the heart condition as heart rate may slow down or arrhythmias may develop. This medication is metabolized and excreted in the liver and kidneys, requiring dosage adjustments and careful observation in patients with liver or kidney dysfunction. Abruptly stopping the medication during use may worsen symptoms, so the dosage should be gradually reduced following a doctor's instructions. This medication is not recommended for pregnant or breastfeeding women, and if breastfeeding, medication should be avoided or nursing should be stopped. The safety and efficacy in children have not been fully established, so it is advisable not to use it in pediatric patients. Elderly patients should start at a lower dose considering possible decreases in liver, kidney, and heart function and administer carefully. In cases of overdose, severe hypotension, bradycardia, heart block, and heart failure may occur, necessitating immediate medical attention. This capsule should not be chewed or opened and must be swallowed whole, and stored in a safe place out of reach of children. It can interact with other medications, so it is essential to inform the healthcare provider of any other medications being taken, and regular monitoring is required.
