This drug is used as an adjunct to diet and exercise to improve blood sugar control in patients with type 2 diabetes. It can control blood sugar as a monotherapy, but if the patient has never used diabetes medications and if monotherapy is insufficient, it may be combined with Metformin. Furthermore, this drug can be added when blood sugar control is difficult with monotherapy of sulfonylureas, Metformin, thiazolidinediones, or insulin. If adequate blood sugar control is not achieved even with various combination therapies, this drug is used additionally to improve blood sugar control. Therefore, this drug is effectively used to improve blood sugar control in various cases.

Dosage and Administration

Adult dosage

This drug is administered at a dose of 100 mg once daily, whether as a monotherapy or in combination therapy, with a maximum daily dose of 100 mg. It can be taken regardless of meals. In patients with renal impairment, dosage adjustments may be necessary based on renal function. - In patients with mild renal impairment (eGFR 60 or above and less than 90), dosage adjustments are not needed. - In patients with moderate renal impairment (eGFR 45 or above and less than 60), dosage adjustments are not needed.

Precautions

This drug can rarely cause serious allergic reactions; hence, if symptoms such as rash, difficulty breathing, or swelling of the face or neck occur within three months of administration, it should be stopped immediately and a doctor should be consulted. Acute pancreatitis, which is inflammation of the pancreas, has been reported; if abdominal pain, vomiting, or persistent abdominal discomfort develops, this drug should be discontinued and medical attention sought immediately. It should not be administered to patients with type 1 diabetes or diabetic ketoacidosis, and it should not be used if there is an allergy to this drug or any of its components. In patients with impaired renal function, the excretion of this drug may be delayed, requiring dosage adjustment; regular renal function tests before and during administration are important. In clinical trials, symptoms such as upper respiratory tract infections, headaches, and nasopharyngitis were reported during the use of this drug, and caution is needed, especially when taken with other antihyperglycemic agents as hypoglycemic symptoms may occur. When used in combination with insulin or sulfonylureas, the risk of hypoglycemia increases, so patients must be aware of hypoglycemic symptoms and adjust the dosage of these drugs as necessary. Rare side effects such as severe arthralgia or bullous pemphigoid have also been reported; if such symptoms occur, the doctor should be informed immediately. Pregnant or breastfeeding women should refrain from taking this drug, and its safety and efficacy have not been established in children and adolescents under 18 years of age. Elderly patients may have impaired renal function, necessitating dosage adjustment and renal function monitoring. In case of overdose, general treatment should be provided, and if necessary, blood dialysis can be performed to remove the drug, so immediate medical assistance should be sought in emergencies. Also, this drug may interact with certain medications, so it is crucial to inform healthcare providers of any other medications being taken.
