This medication is used to treat Torsades de Pointes, a cardiac rhythm disorder. It also helps improve hypomagnesemia associated with acute hypokalemia. It aids in maintaining normal levels through magnesium supplementation in situations of electrolyte imbalance. It provides essential minerals, such as magnesium, during parenteral nutrition, contributing to health maintenance. It is also used to prevent and treat the risk of pre-eclampsia, which is a type of pregnancy-induced hypertension. Overall, this drug improves health by balancing magnesium levels in various conditions.

Dosage and Administration

Adult dosage

For the treatment of Torsades de Pointes, slowly inject magnesium 8 mmol (1.6 g of magnesium chloride) as an IV bolus. Subsequently, maintain a continuous infusion of magnesium 0.012 to 0.08 mmol (2.5 to 16.5 mg of magnesium chloride) per minute. For treating acute hypokalemia with accompanying hypomagnesemia, administer magnesium 24 to 32 mmol (5 to 6.5 g of magnesium chloride) continuously with potassium supplementation over 24 hours.

Children's dosage

In children, administer magnesium 0.1 to 0.3 mmol/kg (20 to 60 mg of magnesium chloride) over 24 hours.

Precautions

When using this medication with glass ampules, there is a risk of glass fragments mixing in, so great care should be taken when cutting the container. Special caution is needed when administering to children or the elderly. The infusion rate should be adjusted slowly when administering this drug, and the patient's condition, including blood pressure, diuretic status, respiration, and reflexes, should be continuously monitored. Regularly check the magnesium levels in the blood and discontinue treatment once normal levels are reached. Co-administration with calcium-containing drugs should be avoided as it may negate the effect. This medication should not be administered to patients with impaired kidney function, severe hypotension, or cardiac and respiratory disorders. Use with special caution in children and patients with heart disease. Pain or warmth may occur at the injection site, and a rapid administration or severe kidney dysfunction may lead to dangerous magnesium overload. Combining with certain medications may cause changes in drug concentration or enhanced effects, so consulting a physician is recommended. For pregnant women, it should only be administered when necessary, as it may affect the fetus or newborn, requiring caution. During lactation, it is advisable to discontinue breastfeeding throughout the treatment period. In case of overdose, serious symptoms such as muscle paralysis, respiratory distress, and cardiac arrest may occur; appropriate treatment must be administered immediately. IV injections should be administered slowly in a professional setting with the patient stable, avoiding mixing with certain solutions as precipitation may occur.

Side effects and when to seek care immediately

Serious side effects

Respiratory distress, respiratory arrest, and cardiac arrest. Severe conditions due to hypermagnesemia (inhibition of knee reflex, muscle paralysis). Hypermagnesemia may pose a life-threatening risk in patients with renal failure.

Mild side effects

Pain at the injection site, warmth, vasodilation. Inhibition of knee reflex, drowsiness, speech difficulties. Blood pressure changes, diuretic effects. Increased plasma magnesium levels. Potential for continuous neuromuscular blockade. Enhanced drug effects may occur with high plasma magnesium levels or dosage.
