View approved original text

This medication helps lower high total cholesterol, LDL cholesterol, apo B, triglycerides, and non-HDL cholesterol levels in patients with primary hypercholesterolemia and mixed dyslipidemia, while increasing HDL cholesterol levels. It is also administered as an adjunct therapy in patients with homozygous familial hypercholesterolemia when other lipid-lowering treatments are not effective or alongside other lipid-lowering therapies. This medication should be used in conjunction with a proper diet, and it is preferable to use when diet alone is insufficient for control. During the treatment of dyslipidemia, it is essential to confirm cholesterol and triglyceride levels through basic blood tests and to identify and treat secondary causes. Treatment may also begin after an early lipid level assessment in patients with acute coronary incidents. Overall, this medication is used to reduce the risk of atherosclerotic vascular disease associated with hypercholesterolemia.

Dosage and Administration

Adult dosage

This medication is administered once daily, regardless of meals. For primary hypercholesterolemia, the dosage range is from 10/10 mg to 10/80 mg per day. An initial dose of 10/10 mg or 10/20 mg daily is recommended. For patients who require more significant reduction in LDL cholesterol (more than 55%), an initial dose of 10/40 mg daily is recommended. After starting treatment or adjusting the dose, lipid levels should be monitored with an interval of at least 2 weeks.

Precautions

There have been reports of a rare side effect called rhabdomyolysis, which can cause severe muscle damage. This may lead to rapid deterioration in kidney function, and if symptoms of severe muscle pain or weakness appear, a healthcare professional should be consulted immediately. Patients with allergies to the ingredients in this medication, severe liver disease, persistently elevated liver enzyme levels of unknown origin, muscle disorders, pregnant or potentially pregnant women, nursing mothers, or patients taking certain antiviral agents should not take this medication. Patients who are alcoholics, have decreased liver function, have kidney issues or hypothyroidism, have a family history of muscle disorders, have previously experienced muscle toxicity from statin medications, elderly patients over 70 years of age, etc., need to exercise special caution when using this medication. This medication may cause side effects such as muscle pain, abdominal pain, elevated liver enzymes, and, rarely, severe allergic reactions, skin rashes, hepatitis, pancreatitis, and other serious adverse reactions. Regular liver function tests should be performed while taking this medication, and if any symptoms of liver damage (nausea, vomiting, fatigue, etc.) appear, use should be discontinued immediately and the healthcare provider informed. If muscle-related symptoms occur, blood tests to check for levels of CPK, an indicator of muscle damage, should be conducted, and if levels are significantly elevated or symptoms are severe, discontinuation of the medication is necessary. This medication may interact with multiple drugs, so if other medications are being taken, it is imperative to inform the doctor or pharmacist, especially when combined with certain antibiotics, antiviral agents, immunosuppressants, and fibrate medications. Pregnant or nursing women should not take this medication, and fertile women should use appropriate contraceptive methods while taking it. The safety and efficacy of this medication for children and adolescents have not been established, and its use is not recommended. Elderly patients are at higher risk for muscle-related side effects, so caution is advised during use, but dose adjustments are generally not necessary. Patients with impaired liver or kidney function should consult a doctor before taking this medication, and patients with severe liver disease should avoid taking it. In cases of overdose, there is no specific antidote, and treatment should be symptomatic, including monitoring liver function and muscle damage indicators.
