This medication is used in combination with diet and exercise to help control blood sugar in patients with type 2 diabetes. It can be given as monotherapy if blood sugar control is difficult, and if a patient has not previously used diabetes medications but struggles with monotherapy, it can be combined with metformin. Additionally, it can enhance effect when used together with sulfonylureas, metformin, thiazolidinediones, or insulin when monotherapy does not provide adequate control. Furthermore, if blood sugar control is still difficult with various combination therapies, this medication can be used additionally to improve treatment. This medication assists in comprehensively improving blood sugar control.

How to take

Adult dose

This medication is administered once daily at a dose of 100 mg either as monotherapy or in combination, with the maximum daily dose of 100 mg. It can be taken regardless of meals. For patients with kidney impairment, dose adjustments according to kidney function are necessary. No dose adjustment is needed in mild and moderate kidney impairment (eGFR 45 or higher, but less than 90). For moderate kidney impairment (eGFR 30 or higher but less than 45), the dose is 50 mg once daily.

Precautions

This medication may rarely cause serious allergic reactions. Such reactions can include anaphylaxis, angioedema, and severe skin conditions. Symptoms may appear within the first 3 months of taking the drug, so caution is necessary. If an allergic reaction is suspected, stop taking the medication immediately and consult a healthcare professional. Moreover, this medication can cause serious side effects, such as acute pancreatitis, which involves inflammation of the pancreas, so if symptoms like abdominal pain, vomiting, or loss of appetite occur during treatment, prompt medical attention is necessary. This medication is not to be used by patients with type 1 diabetes or diabetic ketoacidosis. Also, patients who are allergic to this drug or its components should not take it. For patients with reduced kidney function, dose adjustment is necessary, and regular kidney function tests are important. Common side effects reported in clinical trials include upper respiratory infection, nasopharyngitis, headache, diarrhea, etc., and symptoms of hypoglycemia may occur, so blood sugar management must be monitored closely. Especially when taken with insulin or sulfonylureas, the risk of hypoglycemia may increase, so dose adjustments for these medications may be needed. This medication is not recommended during pregnancy, and it is also advisable for breastfeeding women to avoid taking it. For elderly patients, dose adjustments and monitoring of kidney function are necessary due to the potential for decreased kidney function. In case of overdose, appropriate treatment such as gastrointestinal decontamination and ECG monitoring should be undertaken, and some may be removed by hemodialysis.
