Dosage and Administration

Adult dosage

As sildenafil, the recommended dosage of 25 to 50 mg is to be taken orally about 1 hour before sexual activity, once daily. If necessary, it may also be taken between 4 hours and 30 minutes before sexual activity. Dosage may be adjusted based on efficacy and tolerance.

Precautions

This medication causes vasodilation, which can lower blood pressure; therefore, there is a risk of excessively low blood pressure when taken with nitrate preparations or NO donors. Thus, it is essential to confirm whether such medications are being taken before using this product, and to be cautious not to take them concurrently or after use. Patients with cardiovascular diseases should thoroughly check their heart condition before taking this medication, and those with weak hearts or for whom sexual intercourse is not recommended due to cardiovascular diseases should avoid this medication altogether. This medication can cause transient drops in blood pressure due to vasodilation, particularly in patients with cardiovascular diseases or during sexual intercourse, so caution is advised. For patients with specific conditions such as hypotension, hypertension, heart failure, arrhythmias, or pigmentary retinopathy, the safety and efficacy of this medication have not been fully established, so extreme caution is required. If an erection lasts longer than 4 hours, immediate medical attention should be sought, as untreated cases may lead to penile tissue damage and permanent erectile dysfunction. Patients with severely impaired blood pressure regulation or left ventricular outflow obstruction may be sensitive to the vasodilating effects of this medication, hence careful monitoring is necessary. Co-administration with certain antiviral drugs (e.g., Ritonavir) can significantly increase the plasma concentration of this medication, raising the risk of side effects, necessitating dosage adjustments. The artificial sweetener aspartame included in this product should not be administered to patients with phenylketonuria. Patients allergic to this medication or its components, those regularly taking nitrate preparations, patients with severe cardiovascular diseases, severe liver or kidney failure, individuals under 18 years of age, and women should not use this medication. Additionally, it should not be combined with other erectile dysfunction treatments, and patients with certain cardiovascular symptoms should avoid its use. This medication should be used with caution in patients with anatomical penile deformities, bleeding disorders, the elderly, and those with diabetic retinopathy, and dosages should be adjusted considering the possibility of adverse reactions. Common side effects include headache, flushing, dyspepsia, nasal congestion, dizziness, drowsiness, visual impairment, eye pain, photophobia, color vision changes, conjunctival hyperemia, eye irritation, sensations of altered eye sensation, muscle pain, limb pain, nausea, vomiting, dry mouth, reddening of lips, edema, generalized warmth, facial swelling, itching, rash, hematuria, vasodilation, tachycardia, palpitations, dry eyes, red vision, conjunctival hyperemia, eye swelling, anxiety, hematuria, urinary tract infections, and genital issues. Attention is needed while driving or operating machinery during treatment with this drug, and there may be symptoms of hypotension when combined with blood pressure-affecting medications such as alpha-blockers, necessitating adjustments to dosing intervals and amounts. In case of overdose, appropriate treatment should be provided according to the symptoms, and removal through renal dialysis is difficult. This medication should be kept out of the reach of children and should not be transferred to other containers. Pregnant women or nursing mothers should not use this medication, and it should not be used by patients under 18 years of age. In the elderly, the plasma concentration of this drug may increase, so starting with a low dose is recommended.
