Summary

Main use
Treatment of obesity; weight maintenance; reduction of the risk of weight regain
Who takes it
Adults
Adult dose
Adults: The recommended dose is 120 mg of orlistat per dose, taken orally 3 times daily with a fat-containing meal or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly calorie-restricted diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrates, and protein should be distributed evenly among the three meals. This medicine may be omitted if a meal is skipped or if the meal contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutritional intake. The vitamin supplement should be taken at least 2 hours before this medicine or at least 2 hours after it, such as at bedtime. Taking more than 120 mg per dose 3 times daily does not increase the effectiveness of this medicine. Based on measurements of fecal fat, the effect of this medicine appears 24 ∼ 48 hours after taking it. After treatment is discontinued, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine have not been evaluated for periods exceeding 4 years. Studies have not been conducted in patients with hepatic and/or renal impairment, children under 12 years of age, or elderly patients.

Precautions

This drug should not be used in patients with chronic malabsorption syndrome or biliary obstruction. Patients with an allergic reaction to this drug or its components should also avoid taking it. Women who are pregnant, may become pregnant, or are breastfeeding should not take this drug. There may be liver damage, so if signs of liver dysfunction occur, notify a doctor immediately. It is important to check liver enzyme levels through liver function tests. Patients with hyperoxaluria or calcium oxalate kidney stones should use this drug with caution, as urinary oxalate excretion may increase. Gastrointestinal adverse reactions occur frequently with this drug. For example, oily stools, abdominal distension, flatulence, constipation, and diarrhea may occur, most of which appear within 3 months of starting treatment and subside over time. Patients may discontinue the drug due to adverse reactions, so consult a doctor if symptoms are severe. Taking this drug with a high-fat meal may increase gastrointestinal adverse reactions, so it is recommended to divide daily fat intake evenly across three meals. Additionally, this drug may reduce the absorption of fat-soluble vitamins and beta-carotene, so taking vitamin supplements is recommended. Vitamins should be taken at least 2 hours before or 2 hours after taking this drug. Symptoms of liver dysfunction include decreased appetite, itching, jaundice (yellowing of the skin or eyes), dark urine with light-colored stools, and pain in the upper right abdomen; these symptoms should be reported to medical personnel immediately if they occur. During the use of this drug, patients with diabetes may experience improved blood sugar control, so adjustments to antidiabetic medications or insulin doses may be necessary. Furthermore, excessive weight loss can increase the risk of gallstones, so caution is advised. This drug may interact with various medications, so patients taking especially cyclosporine, anticoagulants, antiepileptics, antidepressants, antipsychotics, or antiretroviral agents should consult their doctor before use. It is recommended not to take it simultaneously with cyclosporine, and patients on anticoagulants should regularly check their blood coagulation status. Women who are pregnant or breastfeeding should not take this drug, and its safety and efficacy have not been confirmed for children under 12 years old. In case of overdose, the patient's condition should be monitored for 24 hours, and the effects of this drug recover quickly.
