Zinc supplementation is essential for patients receiving Total Parenteral Nutrition (TPN). As TPN delivers nutrients without passing through the intestines, some minerals, including zinc, may become deficient. Zinc plays a crucial role in immune function, wound healing, and cell growth; therefore, supplementation is necessary to prevent deficiency. Thus, zinc supplementation during TPN helps maintain patient health and prevent complications.

Dosage and Administration

Adult dosage

Metabolically stable TPN patients: 2.5 to 4 mg/day Adults with fluid losses in the small intestine: additional 12.2 mg/L (amount of fluid loss) or additional 17.1 mg/kg (amount excreted) Acute burn conditions: 4.5 to 6 mg/day Use according to a physician's prescription while checking trace element levels in plasma to guide dosage adjustments.

Precautions

This medication should not be injected directly into the peripheral vein without dilution. This is because administering it in this manner may cause inflammation in the vein or lead to rapid excretion of zinc through the kidneys, which can cause kidney damage. The amount of zinc included in this medication is very small, so toxic symptoms are rarely observed. When mixing this medication with total parenteral nutrition (TPN) solutions, it is advisable to work in a sterile environment to prevent bacterial contamination. Before administration, visually check the solution for any precipitate or discoloration. When administering zinc together with copper, the concentration of copper in the blood may decrease, so the concentrations of both components should be regularly checked to adjust the appropriate dosage. This medication is primarily excreted through the small intestine and kidneys; therefore, caution is advised in patients with poor function of these organs, as zinc may accumulate in the body. The safety of this medication has not been sufficiently established for pregnant women or women of childbearing potential; it should only be administered when the benefits of treatment outweigh the risks. Zinc can cross the placenta and may also be present in breast milk. In cases of overdose, symptoms such as nausea, vomiting, dehydration, electrolyte imbalance, dizziness, abdominal pain, drowsiness, and coordination disorders may occur, and in severe cases, may pose a risk to life. Calcium supplementation may help prevent toxicity due to excessive zinc. This medication should be kept out of the reach of children and should be stored sealed in a dry place at room temperature. Since it does not contain preservatives, any unused medication should be discarded.
