This medication helps patients with type 2 diabetes to manage their blood sugar levels more effectively alongside dietary and exercise therapy. Initially, it can be used alone, and if blood sugar control is difficult even without a history of taking diabetes medications, it may be combined with metformin. Additionally, it is used together when blood sugar control is insufficient with monotherapy of sulfonylureas, metformin, thiazolidinediones, or insulin. Furthermore, it can enhance efficacy when added to different regimens in cases where blood sugar is not well-controlled.

How to take

Adult dose

For monotherapy or combination therapy, 100 mg is administered once daily, with a maximum daily dose of 100 mg. It can be taken regardless of meals. Patients with renal impairment may require dosage adjustments based on renal function. For mildly and moderately impaired renal patients (eGFR 45 or more but less than 90), no dose adjustment is necessary. For moderately impaired renal patients (eGFR 30 or more but less than 45), a dose of 50 mg is administered once daily.

Precautions

This medication can rarely cause serious allergic reactions, so if symptoms such as skin rash, difficulty breathing, or swelling of the face or neck appear within three months of administration, discontinue use immediately and consult a healthcare provider. While taking this medication, symptoms of acute pancreatitis may occur, so if abdominal pain, nausea, or vomiting develops, inform a doctor immediately. This medication should not be used in patients with type 1 diabetes or diabetic ketoacidosis, and it should not be administered to those with known allergic reactions to this drug or its ingredients. Patients with impaired kidney function may require dosage adjustments, so it is important to conduct kidney function tests before administration and check kidney status regularly. Mild respiratory infections, headaches, nasopharyngitis, diarrhea, nausea, and other side effects may occur during this treatment. Particularly, when taken alongside other hypoglycemic agents such as insulin or sulfonylureas, the risk of hypoglycemia is increased, so patients should be alert to the symptoms of hypoglycemia and adjust doses of other medications if necessary. Rare side effects such as severe arthralgia or skin blistering have also been reported, so inform healthcare professionals immediately if experiencing joint pain or skin abnormalities. Pregnant or breastfeeding women are not recommended to take this medication, and safety in children or adolescents under 18 has not been established. Due to potential kidney function decline, elderly patients should be evaluated for kidney function and dosage adjustments are necessary before using this medication. In cases of overdose, general supportive measures should be taken, and if needed, some drugs may be removed via dialysis. There are generally few drug interactions, but those taking digoxin should be monitored closely.
