Improvement of essential hypertension
Essential hypertension helps to control blood pressure within the normal range, reducing the risk of cardiovascular diseases. This medication lowers blood pressure by dilating blood vessels or reducing the burden on the heart, and it is effective for long-term management. Consistent use and lifestyle changes greatly assist in blood pressure control. It is used for the treatment of essential hypertension.
The recommended initial dose is 10mg once daily, taken regardless of meals. If blood pressure is not adequately controlled, increase to 20mg once daily. If further blood pressure reduction is needed, increase to a maximum of 40mg once daily. The blood pressure-lowering effect appears within 2 weeks after starting treatment, with maximum effects observed around 8 weeks. If blood pressure is not controlled with this medication alone, combination treatment with thiazide diuretics such as hydrochlorothiazide may be considered. To improve adherence, it is recommended to take it at the same time each day (e.g., morning) if possible. For patients with possible intravascular volume depletion (e.g., patients on diuretics, especially those with kidney impairment), it is recommended to start treatment at a low dose while monitoring their condition closely. <Elderly Patients> There is no need for dose adjustment when prescribing this medication to elderly patients aged 65 years and over. No overall differences were observed in efficacy and safety between elderly and younger patients, although some elderly may exhibit more sensitive responses. <Patients with Kidney Impairment> The maximum dose for patients with mild to moderate kidney impairment (creatinine clearance of 20-60 mL/min) is 20mg once daily. <Patients with Liver Impairment> There is no need for dose adjustment in patients with mild liver impairment. For patients with moderate liver impairment, the recommended initial dose is 10mg once daily, which can be increased to a maximum of 20mg daily. Experience with severe liver impairment is lacking, so use is not recommended in such patients. <Pediatric Patients> Safety and efficacy in pediatric patients have not been established.

This medication can cause serious harm to the fetus and newborn if used during the second and third trimesters of pregnancy; therefore, it should be discontinued immediately upon confirmation of pregnancy. This medication should not be used in patients with hypersensitivity to it, pregnant or breastfeeding women, patients undergoing kidney dialysis, patients with severe liver dysfunction, patients with biliary obstruction, and certain genetically predisposed patients with lactose intolerance. Patients with reduced blood volume or those taking high doses of diuretics may experience hypotension, so they should be monitored closely, and appropriate measures should be taken as needed. Patients with impaired kidney function, kidney transplant recipients, patients with renovascular hypertension, patients at risk of hyperkalemia, patients with aortic and mitral stenosis, and patients with primary hyperaldosteronism should use this medication cautiously. Dizziness may occur while taking this medication, so it is advisable to avoid driving or operating dangerous machinery. This medication is generally well tolerated, but mild side effects such as dizziness, headache, back pain, and dyspepsia may occur. If symptoms of chronic malabsorption syndrome accompanied by chronic diarrhea and weight loss occur, discontinue the medication immediately and consult a physician. Taking this medication with potassium-sparing diuretics or potassium supplements may lead to elevated potassium levels in the blood, so caution is necessary. The blood pressure-lowering effect may be enhanced when used in combination with other antihypertensives, and the lithium levels may rise and pose a toxicity risk if taken together, so it is best to avoid concurrent use. Taking this medication with non-steroidal anti-inflammatory drugs may worsen kidney function, so hydration and monitoring of kidney function are necessary at the start of treatment. Pregnant or breastfeeding women should avoid using this medication due to the risks to the fetus and newborn; if breastfeeding, it is necessary to stop either breastfeeding or the medication. Safety and efficacy in pediatric patients have not been established, so caution is required. In elderly patients, the concentration of this medication in the body may increase, necessitating dose adjustments and careful monitoring. Overdosage may lead to hypotension and palpitations; if symptoms appear, inform medical personnel immediately and receive appropriate treatment.
