This medication is used to assist in blood sugar control for adults with type 2 diabetes. It is especially administered when prior treatment with diabetes medication has not been undertaken, or when it is difficult to control blood sugar with monotherapy. It can also be used when adequate effect cannot be achieved with monotherapy using metformin, or when replacing the combination of linagliptin and metformin. Moreover, it is used when blood sugar control is difficult even with maximum doses of metformin alongside sulfonylureas or insulin, in order to enhance therapeutic efficacy. If there is a need for combination therapy with empagliflozin, this medication is also administered. This medication improves blood sugar control in individuals with type 2 diabetes in conjunction with diet and exercise.

How to take

Adult dose

The dosage of this medication should be determined considering the current therapy, efficacy, and tolerability of each patient, while not exceeding the maximum recommended doses of linagliptin 2.5 mg and metformin 1000 mg, administered twice daily. This medication should be taken twice daily with meals. 1. If the patient is not currently receiving treatment with metformin: Start with an initial dose of this medication 2.5/500 mg, to be taken twice daily, with one tablet per dose.

Precautions

This medication can cause a rare side effect known as lactic acidosis, which is serious and can lead to death if treatment is delayed. It should not be used in certain patients, for example, those with allergies to its components, type 1 diabetes, lactic acidosis, diabetic ketoacidosis, or severe renal impairment. Patients with compromised renal function, acute infections, dehydration, severe trauma, shock, or acute myocardial infarction—conditions that may affect renal function—should use this medication with caution. It is not recommended for use in pregnant women or those who may become pregnant, as well as nursing mothers. When used with other blood glucose-lowering medications, there is an increased risk of hypoglycemia, so careful monitoring of blood sugar levels is necessary. To reduce the risk of lactic acidosis, renal function should be regularly checked and this medication temporarily stopped in situations that could cause dehydration or acute renal damage. If symptoms such as muscle cramps, abdominal pain, or severe fatigue occur during the use of this medication, it is important to inform a doctor immediately. This medication may reduce vitamin B12 levels, so it is necessary to periodically check vitamin B12 concentrations in the blood during long-term use. In addition, this medication may interact with some drugs, so it is essential to consult a doctor or pharmacist if taking other medications. If iodine-containing contrast agents are used for surgery or radiologic examinations, this medication should be discontinued for a certain period before and after the examination and resumed only after renal function is restored. Elderly patients are prone to renal function decline, so renal function should be frequently checked, and dosage adjustments may be needed when taking this medication. During the treatment, if blistering skin diseases or severe joint pain occurs, consult a physician immediately. Finally, this medication is not recommended for use in children or children under 10 years of age.
