This medication is used as an adjunct to diet and exercise to help control blood sugar levels in adults with type 2 diabetes. It is used in individuals who have not yet received other diabetes medication treatment or in those who have difficulty controlling blood sugar levels with monotherapy. This medication may be administered when sufficient effects are not achieved with monotherapy using metformin or combination therapy with linagliptin and metformin. Additionally, it can be used in conjunction with metformin and sulfonylurea therapy or insulin and metformin therapy when blood sugar control is difficult. This medication can also be used when combination therapy with empagliflozin and metformin is insufficient. It aids in improving blood sugar control as an adjunctive therapy in various combination therapies.

How to take

Adult dose

This medication is administered twice daily with meals. 1. If currently not treated with metformin: The initial dose is this medication 2.5/500 mg, 1 tablet twice daily, which can be titrated to 1 tablet twice daily of 2.5/1000 mg. 2. If adequate blood sugar control cannot be achieved with metformin monotherapy: The initial dose is linagliptin 2.5 mg twice daily.

Precautions

This medication is used for the treatment of diabetes and one of its active ingredients, metformin hydrochloride, may cause a serious side effect known as lactic acidosis; hence, caution is required. This medication should not be administered to certain patients. For example, individuals with allergies to this medication, patients with type 1 diabetes, those with severe renal impairment, patients with acute heart disease or severe infections, individuals with liver function impairment, and pregnant or breastfeeding women should not use this medication. Also, this medication carries a risk of hypoglycemia; thus, blood sugar fluctuations should be closely monitored when used in conjunction with insulin or sulfonylureas, and dosage adjustments may be necessary. Lactic acidosis, although rare, can be a fatal side effect. The risk increases if renal function deteriorates rapidly or if there are severe infections, dehydration, or excessive alcohol consumption. Therefore, in such cases, this medication should be temporarily discontinued, and the patient should consult a doctor immediately. During the use of this medication, regular monitoring of kidney function is necessary, especially for elderly patients or those with reduced kidney function. Patients with poor heart function should also use this medication cautiously and must discontinue it before and after the administration of iodinated contrast agents used in radiologic tests. Acute pancreatitis or skin conditions such as bullous pemphigoid may occur, so if these symptoms arise, the patient should notify a doctor immediately and discontinue the medication. This medication may decrease vitamin B12 levels; therefore, it is advisable to regularly check for vitamin B12 deficiency if used over a long period. Moreover, this medication may interact with various other medications. If the patient is taking other drugs, a consultation with a doctor or pharmacist is recommended, especially when combined with medications affecting kidney function. Pregnant or breastfeeding women are advised not to use this medication, and it is not recommended for children or adolescents. Elderly patients may need dosage adjustments due to declining kidney function and should have regular kidney function tests. In cases of overdose, there is a risk of lactic acidosis, and immediate medical attention is necessary, which may also require hemodialysis. Lastly, this medication should be kept out of reach of children, stored away from moisture, and not transferred to other containers.
