This medication is used in conjunction with diet and exercise to help control blood sugar levels in adult patients with type 2 diabetes. It is administered when there has been no previous treatment with diabetes medications or when blood sugar control is insufficient with monotherapy. It can also be used when sufficient blood sugar control is not achieved with metformin monotherapy. Additionally, this medication is co-administered when switching from the combination of linagliptin and metformin, or when blood sugar is not controlled with maximum doses of metformin and sulfonylureas, or insulin. This medication is also used to improve blood sugar control when empagliflozin and metformin combination therapy is ineffective. Therefore, it is effectively used for blood sugar control in type 2 diabetes requiring combination therapy.

How to take

Adult dose

This medication is taken twice daily with meals. The dosage is determined considering the current therapy of each patient, efficacy, and tolerability within the maximum recommended dosage for each component, which is linagliptin 2.5 mg and metformin 1000 mg, not exceeding twice daily. 1. If not previously treated with metformin: The initial dose is 2.5/500 mg of this medication, taken twice daily, one tablet at a time, which may be increased to 2.5/1000 mg.

Precautions

This medication can cause serious metabolic complications known as lactic acidosis, so it should not be administered to certain patients. For example, patients who are allergic to any component of this medication, type 1 diabetes patients, patients with severe renal impairment, acute heart disease, or liver dysfunction may be at risk if they use this medication. Moreover, it is recommended that pregnant or lactating women avoid taking this medication. When using this medication, care should be taken for symptoms of hypoglycemia, especially when taken in combination with other blood glucose-lowering medications as the risk of hypoglycemia may increase. Due to the risk of lactic acidosis, this medication should be discontinued immediately in situations involving dehydration, severe infections, or acute deterioration of kidney function. Patients with reduced renal function should have regular kidney function tests, and this medication should not be taken if kidney function falls below a certain level. Patients with weakened heart function should also use this medication with caution, and if symptoms of acute pancreatitis or skin disorders like bullous pemphigoid arise, immediate consultation with a doctor is necessary. This medication can lower vitamin B12 levels, so patients on long-term therapy should periodically monitor for vitamin B12 deficiency. Additionally, this medication may interact with other medications, so if other medications are being taken, consultation with a physician or pharmacist is essential. Women who are pregnant or planning to become pregnant are advised to receive insulin therapy instead of this medication for blood glucose control. This medication is not to be used in children under the age of 10, and safety and efficacy have not been established for adolescents aged 10 to 17. Elderly patients should be closely monitored for renal function since renal impairment is common, and typically, the maximum dose is not used. In case of overdose, the risk of lactic acidosis exists, requiring immediate medical treatment, with hemodialysis being the most effective treatment method. This medication should be stored out of reach of children, protected from moisture, and should not be transferred to other containers.
