  • Difficulty in blood sugar control
View approved original text

This medication is used to assist in the blood sugar control of adults with Type 2 diabetes. It is administered as an adjunct to diet and exercise for those struggling with blood sugar control. It can also be used in patients who have not previously been treated with diabetes medications or whose blood sugar control is inadequate with monotherapy. The drug can be used in combination with insulin when blood sugar control is insufficient with insulin and metformin combination therapy, or when replacing linagliptin and metformin combination therapy. Furthermore, in cases where blood sugar control is difficult even when combined with the maximum dose of metformin and empagliflozin, it can be used in combination therapy with empagliflozin. Thus, this drug is used to improve blood sugar control in various treatment scenarios.

Precautions

This medication may cause a serious side effect known as lactic acidosis, and should not be administered to certain patients. In particular, it should not be used in patients with allergies to its ingredients or those with Type 1 diabetes, lactic acidosis, diabetic ketoacidosis, diabetic pre-coma, or severe renal impairment. It is also advisable for patients with acute conditions that may affect kidney function, such as dehydration, severe infections, acute myocardial infarction, and shock, to avoid this medication. Women who are pregnant or breastfeeding should also avoid taking this medication. Care should be taken for the possibility of hypoglycemia when using this drug, especially in combination with medications that have a risk of hypoglycemia such as sulfonylureas or insulin. Lactic acidosis, although rare, is a very serious side effect, and if symptoms such as abdominal pain, muscle cramps, or weakness appear, immediate medical attention should be sought. Patients with impaired kidney function should regularly undergo kidney function tests before taking this medication, and it should not be used in cases of severely impaired kidney function. Patients with poor heart function should be cautious when using this medication, and it should be temporarily discontinued during acute surgeries or administration of radiocontrast media containing iodine. This medication can reduce vitamin B12 levels, so regular checks for vitamin B12 deficiency are needed during long-term use. Additionally, this medication may interact with several other medications, so if any other medications are currently being taken, it is essential to inform healthcare professionals and monitor carefully. Women who are pregnant or planning to become pregnant are advised to use insulin therapy instead of this medication, and the medication should also be avoided during breastfeeding. Safety and efficacy have not been established in children and adolescents, and it is not recommended for them. The elderly are at a higher risk of reduced kidney function, so dose adjustments and monitoring of kidney function are necessary. Overdose may lead to lactic acidosis, so strict adherence to dosage is required, and if any unusual symptoms occur, immediate medical attention should be sought. When storing, keep in a place out of reach of children, avoid moisture, and do not transfer to other containers.
