Summary

Main use
Treatment of gastrointestinal disorders (ulcers and reflux esophagitis), Helicobacter pylori eradication, and gastric acid hypersecretion
Who takes it
Adults
Adult dose
1. Antibiotic combination therapy to prevent recurrence of gastroduodenal ulcers infected with Helicobacter pylori The usual adult dose is 40 mg of pantoprazole twice daily (before breakfast and dinner) for 1 week, in combination with the following antibiotics. Swallow without chewing or crushing. (1) Take amoxicillin 1,000 mg and clarithromycin 500 mg twice daily for 1 week, or (2) Take metronidazole 500 mg and clarithromycin 500 mg twice daily for 1 week, or (3) Take amoxicillin 1,000 mg and metronidazole 500 mg twice daily for 1 week. The metronidazole regimens in (2) and (3) should be used only when regimen 1 is inappropriate. If additional treatment with pantoprazole is indicated to ensure ulcer healing, the dosage recommendations below for duodenal and gastric ulcers should be considered. When combination therapy is not required (e.g., Helicobacter pylori infection is negative), use pantoprazole monotherapy according to the regimens below. 2. Gastric ulcer and reflux esophagitis The usual adult dose is 40 mg of pantoprazole once daily, taken with water before breakfast without chewing or crushing. Healing can usually be achieved within 4 weeks, although the healing period may occasionally vary among patients. If healing is not complete after the initial 4 weeks of treatment, continue treatment for an additional 4 weeks. If the patient has not responded to other treatments, the dose may be doubled (to 80 mg of pantoprazole daily). 3. Duodenal ulcer The usual adult dose is 40 mg of pantoprazole once daily, taken with water before breakfast without chewing or crushing. Healing can usually be achieved within 2 weeks, although the healing period may occasionally vary among patients. If healing is not complete after the initial 2 weeks of treatment, continue treatment for an additional 2 weeks. If the patient has not responded to other treatments, the dose may be doubled (to 80 mg of pantoprazole daily). 4. Zollinger-Ellison syndrome and other pathological hypersecretory conditions Start with an initial dose of 80 mg of pantoprazole daily, then increase or decrease the dose according to measurements of gastric acid secretion. Doses greater than 80 mg should be divided into two daily doses. Temporary increases to 160 mg or more are possible, but should be used only for as long as necessary to adequately control acid secretion. The duration of treatment is not limited, but should be adjusted according to clinical need. In patients with severe liver impairment, reduce the dose so that it does not exceed 20 mg of pantoprazole once daily. In older adults or patients with renal impairment, do not exceed 40 mg of pantoprazole daily. As an exception, older adults may receive this drug at a dose of 40 mg twice daily (before breakfast and dinner) for 1 week as part of the antibiotic combination therapy described above. Swallow without chewing or crushing. Antibiotic combination therapy should not be used in patients with moderate to severe hepatic/renal impairment. There are insufficient data on the efficacy and safety of this combination therapy.

This medication is used as part of antibiotic combination therapy to prevent recurrence of gastric and duodenal ulcers infected with Helicobacter pylori. It is also effective in treating duodenal ulcers and gastric ulcers, as well as moderate to severe reflux esophagitis. It aids in the treatment of pathological conditions where gastric acid is excessively secreted, such as Zollinger-Ellison syndrome. This medication plays an important role in alleviating symptoms and preventing recurrence of gastric and duodenal-related disorders. In summary, this medication is used to treat conditions related to gastric ulcers, duodenal ulcers, reflux esophagitis, and excessive gastric acid.

Precautions

This medication should not be used in patients with moderate to severe impairment of liver or kidney function due to insufficient clinical experience with its safety or efficacy. Patients who have had hypersensitivity reactions to this medication or any of its ingredients, or to benzimidazoles, should not be administered this medication, as hypersensitivity reactions can lead to anaphylactic shock, angioedema, bronchospasm, acute interstitial nephritis, urticaria, etc. In particular, patients allergic to penicillin or macrolide antibiotics, as well as those taking certain medications (terfenadine, cisapride, pimodivir, astemizole, atazanavir, nelfinavir, rilpivirine) should avoid using this medication. It is also safer for women who are in the early stages of pregnancy or who may become pregnant, as well as for nursing mothers, to avoid taking this medication. Since this medication suppresses gastric acid secretion, long-term use may increase the number of bacteria in the stomach, raising the risk of infections and potentially causing electrolyte imbalances such as increased risk of fractures and hypomagnesemia. Therefore, regular monitoring is necessary if used for more than one year, and if symptoms persist or adverse reactions occur, immediate consultation with a healthcare provider is required. Additionally, this medication may interact with other drugs, so it is essential to inform healthcare professionals of any ongoing treatments, especially anticoagulants, antivirals, antibiotics, and immunosuppressants. When taking this medication, it should be swallowed whole without chewing or crushing and stored out of reach of children in a safe place. There are no specific symptoms known for overdose, but if unusual symptoms appear, appropriate treatment should be obtained by visiting a medical institution immediately. Finally, there is insufficient experience in using this medication in children and adolescents, so it is advisable to avoid use in patients under 18 years of age.
