Improvement of essential hypertension
Essential hypertension is a disease treated to control blood pressure within the normal range. This medication is used to lower blood pressure and reduce the risk of cardiovascular diseases caused by hypertension. It helps prevent serious complications such as stroke and myocardial infarction through the management of hypertension. Additionally, it contributes to the protection of kidney function by maintaining stable blood pressure levels. Thus, it is an effective drug for the treatment of essential hypertension, used for blood pressure control and prevention of complications.
The recommended initial dose for adults is 10mg once daily, taken regardless of meals. If blood pressure is not adequately controlled, the dose can be increased to 20mg once daily, and a maximum dose of 40mg can be administered. The blood pressure-lowering effect appears within 2 weeks of starting treatment and reaches its maximum effect in about 8 weeks. If blood pressure control is insufficient with mono-therapy, thiazide diuretics such as hydrochlorothiazide may be added. To improve adherence, it is recommended to take the medication at the same time every day (e.g., in the morning). For patients with potential intravascular volume depletion (e.g., patients on diuretics, especially those with renal impairment), treatment should begin at a low dose with careful monitoring. <Elderly patients> No dose adjustment is necessary when administering this medication to elderly patients aged 65 and above. While no significant differences in efficacy and safety have been observed between elderly and younger patients, it cannot be ruled out that some elderly patients may demonstrate a more sensitive response. <Renal impaired patients> For patients with mild to moderate renal impairment (creatinine clearance 20-60mL/min), the maximum dose is 20mg once daily. <Liver impaired patients> There is no experience with this medication in patients with liver impairment, hence its use is not recommended. <Pediatric patients> The safety and efficacy in pediatric patients have not been established.

This medication can cause serious harm or death to the fetus and newborn if administered to women in the second or third trimester of pregnancy, so if pregnancy is confirmed, use should be discontinued immediately. This medication should not be used by certain patients, including those who are allergic to this drug or its ingredients, pregnant or breastfeeding women, patients undergoing kidney dialysis, those with severe liver dysfunction, patients with biliary obstruction, lactose-intolerant patients, those with certain diabetes conditions, and patients with severely impaired kidney function. Patients who have a reduced blood volume or salt might experience low blood pressure when starting this medication, so such patients should be monitored closely when beginning treatment. If low blood pressure occurs, the patient should be laid down, and if necessary, normal saline should be administered intravenously. This medication can affect kidney function, therefore, patients with weak kidney function or kidney transplant recipients should regularly check their kidney function and electrolyte status through blood tests, and it is not recommended for patients with severe kidney impairment. Patients with problems in kidney blood vessels or those at risk of hyperkalemia should also be cautious, and due to the possibility of dizziness from blood pressure reduction, driving or operating dangerous machinery should be avoided. Mild side effects such as dizziness, headache, indigestion, muscle pain, and rashes may occur during the use of this medication, and in rare instances, vascular edema or liver function abnormality has been reported. Caution should be taken when this drug is taken concurrently with other antihypertensive drugs, potassium-sparing diuretics, or potassium supplements, as this may cause dangerously high potassium levels in the blood, and co-administration with lithium preparations can lead to lithium toxicity. Taking this drug with non-steroidal anti-inflammatory drugs may increase the risk of decreased kidney function, so it is necessary to ensure adequate fluid intake and monitor kidney function at the beginning of treatment. Taking this drug during pregnancy can lead to serious side effects for the fetus, so if a pregnancy is confirmed, the drug should be stopped immediately, and pregnant women should have regular checks on the fetus’s condition. It is recommended that breastfeeding women discontinue either the drug or breastfeeding, as this medication can potentially be transferred to breast milk. Safety for pediatric use has not been established, so its use is not recommended. Caution is required for the elderly as drug concentration in the body may increase.
