Essential hypertension is hypertension that occurs without a specific cause, and it is used to lower blood pressure and prevent cardiovascular complications.

How to take

Adult dose

Adults should take 10 mg once daily, regardless of meals. If blood pressure is not adequately controlled, the dose may be increased to 20 mg once daily; if further blood pressure reduction is needed, the maximum dose may be increased to 40 mg once daily. Blood pressure-lowering effects appear within 2 weeks of starting treatment, and the maximum effect is observed after about 8 weeks. If blood pressure is not controlled with monotherapy, thiazide diuretics such as hydrochlorothiazide may be added.

Child dose

Safety and efficacy in children have not yet been established.

Precautions

This medication can cause serious harm or death to the fetus and newborn if taken during the second and third trimesters of pregnancy; therefore, it should be discontinued immediately upon confirmation of pregnancy. Patients allergic to any ingredients in this medication, pregnant or nursing women, patients undergoing renal dialysis, patients with severe liver dysfunction, patients with biliary obstruction, etc., should not take this medication. Patients with insufficient blood volume or sodium may experience hypotension at the initial dosage, so caution is necessary, and appropriate treatment should be received if symptoms occur. This medication may affect kidney function, so patients with impaired kidney function should have their kidney status checked through regular blood tests. There is a risk of hyperkalemia; therefore, patients with heart disease or kidney disease in particular should frequently check their serum potassium levels. Patients with aortic or mitral stenosis, obstructive or hypertrophic cardiomyopathy should take this medication with special caution. This medication may cause dizziness; therefore, it is advisable to avoid dangerous tasks such as working at heights or driving vehicles. This medication generally has few side effects and good tolerability; however, symptoms such as dizziness, headache, dyspepsia, muscle pain, and skin rash may occur. Taking this medication during pregnancy increases the risk of fetal kidney dysfunction, oligohydramnios, and craniofacial malformation; therefore, women planning to conceive or those who are pregnant should consult with healthcare providers. Nursing mothers should be aware that this medication may be transmitted through breast milk and could cause side effects in infants; therefore, they should either stop nursing or discontinue the medication. Safety and effectiveness in children have not yet been established, so it should be administered to children with caution. Elderly patients may have increased blood concentrations of the drug; therefore, caution is required when administering this medication. Interactions may occur if this medication is taken with potassium-sparing diuretics, potassium supplements, lithium, or non-steroidal anti-inflammatory drugs, so healthcare providers should be informed. In cases of overdose, hypotension and palpitations may occur, and appropriate treatment should be provided depending on the symptoms. This medication has not been reported to be carcinogenic or genotoxic even with long-term use; however, rare side effects such as liver dysfunction or angioedema may occur; therefore, regular monitoring is necessary.
