This medication is used to treat essential hypertension. Additionally, it helps to reduce the risk of myocardial infarction, stroke, and death from cardiovascular diseases in patients over 55 years old who are at high risk for cardiovascular diseases. It is particularly useful in patients with a history of coronary artery disease, peripheral artery disease, stroke, or high-risk diabetes. It is effective as an alternative therapy when ACE inhibitors cannot be used. This medication aids in managing hypertension and protecting cardiovascular health simultaneously, improving the overall health of patients.

How to take

Adult dose

For the treatment of essential hypertension, the recommended dosing of telmisartan is 40 mg orally once daily, which may be effective at 20 mg once daily in some patients. If necessary, the dose may be increased to 80 mg once daily. For the purpose of reducing the risk of cardiovascular disease, an oral dose of 80 mg once daily is recommended. In patients with hepatic impairment (mild to moderate), the dose should not exceed 40 mg once daily.

Precautions

This medication can pose serious risks of harm or death to the fetus and newborn during pregnancy; therefore, it should be discontinued immediately if pregnancy is confirmed. It should not be used in individuals allergic to this drug or its components, in pregnant or breastfeeding women, patients with severe liver impairment, patients with hereditary angioedema, or patients with severe renal impairment. Elderly patients, patients with high potassium levels, patients with liver impairment, patients with heart valve disease or hypertrophic cardiomyopathy, gastrointestinal disease patients, and those with renal artery hypertension should take this medication with caution. During treatment with this drug, side effects such as infections, blood abnormalities, allergic reactions, hyperkalemia, hypoglycemia, dizziness, hypotension, dyspepsia, liver dysfunction, skin rashes, myalgia, and renal function impairment may occur. Any unusual symptoms should be discussed with a doctor immediately. Particularly, blood pressure may drop too low, leading to symptoms of shock or fainting, so careful monitoring is needed, and appropriate treatment should be applied if necessary. Patients with reduced kidney function should regularly check potassium and creatinine levels through blood tests. Dialysis patients may take the medication without dose adjustments, but their condition should be closely monitored. Patients with reduced blood volume or those taking diuretics are at risk for hypotension, so it is essential to normalize their condition before administration. This medication may enhance blood pressure-lowering effects when taken with other antihypertensives, so consultation with healthcare professionals is necessary. Particular caution is needed when used concurrently with lithium, digoxin, ramipril, potassium-sparing diuretics, and non-steroidal anti-inflammatory drugs, as the risk of side effects may increase. Women who are pregnant or planning to become pregnant should avoid using this medication, and breastfeeding women are also advised to avoid it. Safety and effectiveness have not been established in children under 18 years of age; therefore, it is not recommended for this population. In case of overdose, hypotension, dizziness, or acute renal failure may occur, requiring immediate medical attention. This medication should be kept out of the reach of children and should not be transferred to other containers. Caution should be taken while driving or operating machinery as drowsiness or dizziness may occur.
