Precautions

This medication should not be used by patients who are allergic to this drug or its ingredients. Caution should be exercised in patients with impaired kidney function, as neurological symptoms may be more easily manifested. Patients with impaired liver function should be administered carefully due to the risk of further deterioration of liver function. In infants and children, it should be used minimally and only when necessary, particularly in newborns and low-birth-weight infants where safety has not been established, requiring stricter precautions. Elderly patients may have decreased kidney function, which can lead to the medication remaining in the body longer, hence increasing the risk of adverse reactions; therefore, dosage and intervals should be adjusted and adequate hydration is needed. While using this medication, serious allergic reactions such as shock or anaphylaxis may occur; if symptoms such as low blood pressure, difficulty breathing, or chest tightness manifest, administration should be stopped immediately and appropriate treatment should be sought. If symptoms of phototoxicity, skin rash, fever, blisters, or urticaria appear, discontinue the medication and consult a physician. Serious skin reactions such as hair loss, severe dermatitis, Stevens-Johnson syndrome, and Lyell syndrome may occur, requiring close monitoring of skin condition. Neurological symptoms may include altered consciousness, disorientation, decreased concentration, aggression, hallucinations, convulsions, drowsiness, tremors, anxiety, and insomnia, particularly in elderly or patients with impaired kidney function. It may affect the kidneys, leading to renal failure, hematuria, proteinuria, so renal function should be regularly monitored. Hematological and lymphatic system side effects may include leukopenia, thrombocytopenia, bleeding, and anemia, hence blood tests should be conducted to check the condition. Liver dysfunction, hepatitis, or jaundiced symptoms may occur, so periodic liver function tests are recommended. Gastrointestinal symptoms may include gastritis, nausea, vomiting, diarrhea, constipation, and abdominal pain; these symptoms should be consulted with a doctor. Generally, this medication should be administered for 5 days for herpes simplex and chickenpox and for 7 days for shingles; if symptoms do not improve, alternative treatments should be considered. It is most effective to start treatment as early as possible when symptoms appear; for shingles, administration should begin within 5 days of skin symptom onset, and for chickenpox within 3 days. Patients with compromised immune function may consider intravenous administration. During administration, especially at high doses, it is important to maintain adequate hydration to prevent dehydration and reduce the risk of nephrotoxicity. In the treatment of recurrent genital herpes, the decision to continue treatment should be evaluated after one year. This medication may cause altered consciousness, requiring caution when driving or operating hazardous machinery, particularly in patients with impaired kidney function. There may be interactions with other medications, especially those affecting kidney function, such as probenecid, cimetidine, theophylline, and mycophenolate mofetil, so patients should consult with a doctor or pharmacist if taking these. Pregnant women or those who may become pregnant should carefully consider the risks and benefits of treatment before using this medication; since this medication is excreted in breast milk, nursing should be stopped during treatment. In the case of overdose, renal function deterioration, neurological symptoms may occur, so immediate consultation with medical personnel is necessary, and if needed, treatments such as dialysis should be pursued.
