Treatment of essential hypertension and reduction of cardiovascular disease risk
This medication is used to effectively lower blood pressure in patients with essential hypertension. It also reduces the risk of death from cardiovascular disease in patients aged 55 and older who are at high risk of myocardial infarction, stroke, and cardiovascular disease and who cannot take ACE inhibitors. It is particularly useful for patients with a history of coronary artery disease, peripheral artery disease, stroke, or transient ischemic attack, or those with evidence of damage to end organs who are at high risk for diabetes. Therefore, it plays an important role in the management of hypertension and the prevention of cardiovascular disease.
For essential hypertension, Telmisartan 40 mg should be taken orally once daily with water, and it can be taken regardless of meals. Depending on the patient, 20 mg once daily may also be effective, and if necessary, the dose can be increased up to 80 mg once daily. The maximum antihypertensive effect occurs 4 to 8 weeks after dose adjustment. For the purpose of reducing cardiovascular disease risk, once daily administration of 80 mg is recommended.
This medication can cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters, so it should be discontinued immediately upon confirmation of pregnancy. Patients who are allergic to this medication or its ingredients, pregnant or breastfeeding women, patients with severe liver disease or biliary excretion disorders, patients with hereditary angioedema, and patients with diabetes or moderate to severe renal impairment should not take this medication. Elderly patients, those with hyperkalemia, patients with reduced liver function, patients with heart valve disease or ischemic heart disease, patients with gastrointestinal disease, patients with renal artery hypertension, and patients with severe renal impairment should take this medication with caution and their condition should be monitored regularly as directed by a physician. This medication may rarely cause side effects such as hypotension, dizziness, muscle pain, dyspepsia, skin rash, and angioedema. If any unusual symptoms occur, immediate consultation with a healthcare professional is necessary. Particularly in the case of angioedema, careful observation is needed, as swelling of the face, lips, or tongue may occur along with difficulty breathing, and the risk of hyperkalemia and renal function impairment may arise, making regular blood tests important. The risk of side effects may increase if this medication is taken with other blood pressure medications, diuretics, or potassium-sparing agents, so consultation with a doctor is essential before taking it. Patients scheduled for surgery should preferably stop taking this medication 24 hours before the procedure. This medication is not recommended for children and adolescents under 18 years of age due to insufficient safety and efficacy data. In case of overdose, symptoms such as hypotension, dizziness, and acute renal failure may occur, so it is important to adhere to the prescribed dosage, and if any unusual symptoms arise, immediate hospital visit is advised. This medication should be stored out of reach of children, and it is advisable to open the packaging just before use to avoid moisture exposure. Caution is necessary when driving or operating dangerous machinery, as dizziness or fainting may occur. Finally, women who are pregnant, planning to become pregnant, or breastfeeding should consult a physician before taking this medication.
