Improvement of symptoms of acute coronary syndrome and atherosclerosis; prevention of thromboembolism in atrial fibrillation
This medication is used to improve symptoms caused by atherosclerosis in patients with ischemic stroke, myocardial infarction, and peripheral arterial disease. It also applies to patients with acute coronary syndrome, particularly those with unstable angina or non-Q wave myocardial infarction, and is effective in reducing the risk of cardiovascular abnormalities in patients who have undergone drug therapy, coronary intervention, or coronary artery bypass surgery. Additionally, it helps reduce the risk of thromboembolism such as strokes in patients with atrial fibrillation who have one or more vascular risk factors, where vitamin K antagonists are not appropriate and bleeding risk is low. Overall, this medication is used to effectively manage the health of patients by reducing various vascular-related risks and improving symptoms.
○ Adults 1. In patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer 75 mg of clopidogrel orally once daily. 2. In patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), initiate treatment on the first day with a loading dose of 300 mg once daily, then administer a maintenance dose of 75 mg orally once daily. Aspirin 75 ~ 325 mg must be administered concomitantly once daily. 3. In patients with atrial fibrillation, administer 75 mg of this medicine orally once daily. Aspirin 75 ~ 100 mg must be administered concomitantly once daily. This medicine may be taken with or without food.
This drug is designed to prevent platelet aggregation in the blood, thus preventing thrombus formation. Its efficacy may be reduced in individuals with genetically low functioning of a specific enzyme (CYP2C19), in which case alternative treatment methods or medications should be considered. Additionally, concomitant use of this drug with medications that activate the enzyme may cause increased blood levels of this drug, raising the risk of bleeding, and such combinations are not recommended. Patients with bleeding or severe liver damage, those allergic to this drug or its components, breastfeeding women, and patients with certain genetic disorders should not use this drug. Patients with a high risk of bleeding or those scheduled for surgery should exercise caution when taking this drug and may need to discontinue it 5 to 7 days prior to surgery. The risk of bleeding may be amplified when this drug is taken with other medications that increase bleeding risk, so caution is warranted. Patients should consult a physician immediately if symptoms of bleeding or adverse reactions occur during the use of this medication, and necessary tests including blood tests should be conducted. While the risk to the fetus during pregnancy is low, the risk of bleeding may increase, requiring attention during childbirth or when undergoing nerve blocks. In cases of breastfeeding, it is unclear whether this drug is secreted in breast milk, so one should consult a physician about whether to continue breastfeeding or to discontinue the medication. The safety and efficacy in pediatric use have not been established, hence it is not recommended for children. Elderly patients or those with impaired renal and hepatic function may have a higher risk of bleeding and require dosage adjustments and monitoring of their condition. In cases of overdose, bleeding time may be prolonged, and while there is no specific antidote, platelet transfusion may reverse the effects. This medication should be kept out of reach of children, and care should be taken as transferring it to another container may alter its quality.
