Improvement of atherosclerotic symptoms in ischemic stroke, myocardial infarction, and reduction of the risk of stroke and thromboembolism.
This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. Additionally, in patients with acute coronary syndrome who have experienced unstable angina or non-ST elevation myocardial infarction, it improves symptoms associated with atherosclerosis such as cardiovascular-related death, myocardial infarction, and stroke following drug treatment, percutaneous coronary intervention, or coronary artery bypass grafting. Finally, it helps to reduce the risk of stroke and thromboembolism in adults with one or more vascular risk factors when the administration of vitamin K antagonists is not appropriate. In summary, this medication is effective in improving vascular health in patients with various cardiovascular and cerebrovascular diseases and reducing the risk of serious complications.
For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, Clopidogrel is administered orally at a dosage of 75 mg once daily. For patients with acute coronary syndrome, a loading dose of 300 mg should be administered orally once on the day of starting treatment, followed by a maintenance dose of 75 mg once daily. At this time, aspirin 75–325 mg should be co-administered once daily. For patients with atrial fibrillation, administer 75 mg once daily.
This medication helps prevent the aggregation of platelets, allowing blood to flow more easily. Individuals with a genetic deficiency in the CYP2C19 enzyme may have reduced effectiveness of this medication, in which case treatment options should be reconsidered. Moreover, concurrent use with drugs that increase the activity of the CYP2C19 enzyme may elevate the blood concentration of this medication, increasing the risk of bleeding, so co-administration is not advised. Patients who are allergic to this medication or its components, those with bleeding issues, severe liver dysfunction, those who are breastfeeding, and those with genetic conditions such as lactose intolerance should not take this medication. Caution should be exercised in patients at high risk of bleeding or those with bleeding disorders, and if surgery is planned, the physician should be consulted to discontinue the medication 5 to 7 days prior to the procedure. This medication may increase the likelihood of bleeding when taken with other drugs that also pose bleeding risks, so attention is warranted. Patients should inform the hospital immediately of any bleeding symptoms or unusual signs while taking this medication and regular blood tests are recommended. This medication primarily inhibits platelet function and can prolong bleeding time, which may result in bleeding taking longer than usual to stop. Taking this medication generally does not have a significant effect on driving or operating machinery, but caution is advised as individual responses may vary. While no significant risks to the fetus during pregnancy have been reported, a higher risk of bleeding during delivery may occur, hence discontinuation before childbirth is recommended. In breastfeeding cases, since this medication may pass into breast milk, patients should consult their physician about whether to continue breastfeeding or discontinue the medication. Usage in children is not recommended as safety and efficacy have not been established. The elderly may have reduced physical function leading to increased bleeding risks, so dosage adjustments and monitoring of conditions are necessary. In case of overdose, the risk of bleeding may increase, and immediate medical attention is required as there is no specific antidote; platelet transfusion may be necessary for severe bleeding. This medication should be stored out of reach of children and should not be transferred to other containers.
