Adult dose
For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer as clopidogrel 75 mg orally once daily. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), initiate treatment with a loading dose of 300 mg orally once daily on the first day, followed by a maintenance dose of 75 mg once daily. This medication is used to improve symptoms due to atherosclerosis in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. It also helps protect patients with acute coronary syndrome, especially those who have received or will receive drug treatment or coronary intervention, from cardiovascular complications and aids in the improvement of atherosclerotic symptoms. In addition, in patients with atrial fibrillation who have vascular risk factors and where vitamin K antagonists are not suitable and the risk of bleeding is low, it is effective in reducing the risk of stroke and thromboembolism. Thus, this medication shows positive effects in the prevention and treatment of various cardiovascular and cerebral vascular diseases.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer as clopidogrel 75 mg orally once daily. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), initiate treatment with a loading dose of 300 mg orally once daily on the first day, followed by a maintenance dose of 75 mg once daily. In this case, aspirin 75-325 mg should be co-administered once daily.

Precautions

This medication helps prevent platelets from aggregating, allowing blood to flow more freely. The drug is converted into its active form by specific enzymes (CYP2C19) in the liver, and patients with genetically reduced enzyme activity may experience diminished efficacy. Therefore, if such individuals are identified, other treatment options should be considered. Using this medication with drugs that activate it can increase the drug concentration and possibly elevate the risk of bleeding, so co-administration is not advisable. Patients with allergies to this medication or its components, those with bleeding disorders, severe liver disease, nursing mothers, and those with certain genetic conditions like lactose intolerance should not take this medication. Patients at high risk for bleeding, those with compromised liver or kidney function, recent stroke, persistent hypertension, elderly patients, and underweight individuals should use this medication cautiously. Additionally, patients who have had allergies to drugs in the same class should exercise caution. As this medication can increase the risk of bleeding, patients should immediately inform their doctor of any bleeding symptoms or abnormalities. If surgery or invasive procedures are planned, it is necessary to stop taking this medication 5-7 days before the surgery, and particular caution should be taken when consumed with other drugs that increase bleeding risk. While taking this medication, careful observation for symptoms of bleeding should be conducted, and blood tests may be necessary. This medication may have side effects including gastrointestinal disturbances, skin rash, headache, dizziness, fatigue, dyspepsia, and diarrhea, and rarely serious issues like severe bleeding or decreased blood cells may occur. During pregnancy, there is a low risk to the fetus; however, caution should be exercised due to the risk of bleeding at delivery, and if breastfeeding, careful consideration should be taken as the drug may pass into breast milk. In cases of overdose, the risk of bleeding may increase, and there are no specific antidotes, so treatments like platelet transfusion may be necessary if severe bleeding occurs. The safety and efficacy of this medication in children have not been established. Due to the potential for decreased renal and liver function in the elderly, careful dose adjustment and monitoring are required. This medication can interact with various other drugs, therefore patients must inform their doctor or pharmacist if they are taking any other medications. In particular, caution is warranted when ingested with drugs that enhance bleeding risk or certain gastrointestinal medications, anticoagulants, some antibiotics, and psychiatric medications. While taking this medication, caution may be necessary when driving or operating machinery; however, it is generally known to have minimal effects on psychological functions. Finally, this medication should be stored out of reach of children and kept in its original container for safety.
