View approved original text

This medication is used to help control blood sugar in patients with type 2 diabetes. It is particularly effective for patients who are also following a dietary regimen and exercise, as it contains both pioglitazone and metformin. This assists in maintaining blood sugar levels more steadily. Regular administration helps improve blood sugar control.

Dosage and Administration

Adult dosage

The initial dosage of this medication should be based on the patient's current treatment regimen. For patients whose blood sugar is not adequately controlled with monotherapy of metformin or pioglitazone, the dosage should be based on the currently administered dose of metformin or pioglitazone, starting with one tablet of this medication once a day, and gradually adjusting the dose based on the appropriateness of the treatment response. For patients transitioning from combination therapy with pioglitazone and metformin to this medication, patients may start at the existing doses of pioglitazone and metformin.

Precautions

This medication contains two main ingredients, each contributing to blood sugar control. The first ingredient is metformin, and there is a risk of decreased kidney function or acute kidney injury during treatment, requiring regular kidney function tests. Metformin can rarely cause a serious metabolic condition known as lactic acidosis, which may present with symptoms such as muscle pain, fatigue, and difficulty breathing; if these symptoms occur, medical personnel should be notified immediately. Additionally, it is advisable that patients with impaired liver function or those who consume alcohol excessively avoid this medication. The second ingredient is pioglitazone, which may worsen heart failure, so care should be taken when administered to patients with heart disease or heart failure symptoms. This medication should not be used in patients with severe heart failure, and swelling due to fluid retention may occur; if symptoms develop, consultation with medical personnel is necessary. This medication is not recommended for patients with a history of bladder cancer or active bladder cancer. Symptoms such as urinary disturbances or hematuria should be reported to a physician immediately. Women who are pregnant or breastfeeding should not take this medication, and safety and efficacy have not been established in pediatric patients. In elderly patients, kidney function may be diminished, requiring careful adjustment of the initial dose and regular kidney function testing. During treatment, there may be interactions with other medications, so patients should inform medical personnel about all medications they are taking, especially those that may affect kidney or liver function. Before undergoing surgery or procedures involving iodine contrast agents, medication should be temporarily discontinued, and should be resumed once kidney function has been confirmed to be normal. Finally, patients taking this medication should regularly monitor their blood sugar and HbA1c levels to assess treatment effects, and due to the risk of vitamin B12 deficiency, it is advisable to check this through blood tests.
