Treatment of overactive bladder symptoms (urge urinary incontinence, frequency, etc.)
This medication is used to treat symptoms of overactive bladder such as urgency incontinence, frequent urination, and urge incontinence. These symptoms are caused by excessive contraction of the bladder, making it difficult to hold urine. This drug helps to relieve the excessive contractions of bladder muscles to improve these symptoms. As a result, it can reduce discomfort in daily life for patients and improve urinary control. It effectively manages symptoms related to overactive bladder, thereby improving quality of life.
The recommended dose for adults (including elderly patients) is 5mg once daily. If necessary, the dose may be increased to a maximum of 10mg once daily. This medicine may be taken with or without food and should be swallowed whole with water. Dosage adjustment is required for the following patients. 1. Patients with renal impairment: No dosage adjustment is necessary in patients with mild or moderate renal impairment (creatinine clearance>30ml/min). Use caution in patients with severe renal impairment (creatinine clearance≤30ml/min), and do not exceed 5mg once daily. 2. Patients with hepatic impairment: No dosage adjustment is necessary in patients with mild hepatic impairment. Use caution in patients with moderate hepatic impairment (Child-Pugh class B), and do not exceed 5mg once daily. 3. Patients receiving potent cytochrome P450 3A4 (CYP3A4) inhibitors: Limit the maximum daily dose to 5mg in patients receiving ketoconazole or other potent CYP3A4 inhibitors (e.g., ritonavir, nelfinavir, itraconazole).
This drug should not be used in patients with urinary retention symptoms, severe gastrointestinal disorders, myasthenia gravis, or narrow-angle glaucoma. Additionally, patients who have hypersensitivity to this medication, are undergoing hemodialysis, or have severe liver or kidney impairment should avoid using it. Caution is advised for patients who are at risk of urinary retention or have obstructive gastrointestinal diseases or reduced gastrointestinal motility. Patients with severe kidney impairment or moderate liver impairment should limit the maximum daily dose to 5mg, and dosage adjustments are required when used in conjunction with strong CYP3A4 inhibitors. This medication may cause side effects such as dry mouth, constipation, blurred vision, urinary retention, and dry eyes, particularly at high doses where the incidence of adverse reactions is higher. Rarely, severe allergic reactions, such as angioedema and anaphylaxis, have been reported, so treatment should be discontinued immediately and medical attention sought if symptoms occur. Patients with cardiac risk factors, such as QT prolongation syndrome or hyperkalemia, may experience cardiac rhythm abnormalities, warranting caution. Drowsiness or blurred vision may occur while driving or operating machinery, so caution is advised. This drug can increase effectiveness or worsen side effects when used with other anticholinergic agents, requiring caution when used in combination. It may also reduce the effectiveness of drugs that increase gastrointestinal motility, so be cautious when taken together. This drug is metabolized by CYP3A4 enzymes, thus dosage adjustments may be necessary when used with strong CYP3A4 inhibitors or inducers. Safety during pregnancy has not been established, so caution should be taken when used, and breastfeeding women are advised not to use this drug. Safety and efficacy in pediatric patients have not been confirmed, so it should not be used in this population. In cases of overdose, symptoms such as central nervous system symptoms, blurred vision, thirst, and dizziness may occur, and appropriate emergency treatment should be sought if necessary.
