Treatment of overactive bladder symptoms (frequency, urinary urgency, etc.)
This medication is used to treat symptoms of overactive bladder such as urge urinary incontinence, frequency, and urgency. Overactive bladder is characterized by a sudden, uncontrollable urge to urinate, and it helps reduce these symptoms to alleviate discomfort in daily life. It aids in improving the quality of life of patients by controlling the frequency and urgency of urination.
The recommended dose for adults (including elderly patients) is 5mg once daily. If necessary, the dose may be increased to a maximum of 10mg once daily. This medicine may be taken with or without food and should be swallowed whole with water. Dosage adjustment is required for the following patients. 1. Patients with renal impairment: No dosage adjustment is necessary in patients with mild or moderate renal impairment (creatinine clearance>30ml/min). Use caution in patients with severe renal impairment (creatinine clearance≤30ml/min), and do not exceed 5mg once daily. 2. Patients with hepatic impairment: No dosage adjustment is necessary in patients with mild hepatic impairment. Use caution in patients with moderate hepatic impairment (Child-Pugh class B), and do not exceed 5mg once daily. 3. Patients receiving potent cytochrome P450 3A4 (CYP3A4) inhibitors: Limit the maximum daily dose to 5mg in patients receiving ketoconazole or other potent CYP3A4 inhibitors (e.g., ritonavir, nelfinavir, itraconazole).
This medication should not be used in patients with urinary retention symptoms or serious gastrointestinal problems such as toxic megacolon, myasthenia gravis patients, narrow-angle glaucoma patients, and those allergic to this medication or any of its components. Patients undergoing hemodialysis or those with severe hepatic impairment or severe renal impairment should also avoid this medication. Patients with certain metabolic disorders such as galactose intolerance should avoid this medication as well. This medication should be administered with caution to patients at risk of urinary retention or those with bladder outlet obstruction, patients with gastrointestinal obstructive diseases, patients at risk of decreased gastrointestinal motility, and patients with severe renal impairment or moderate hepatic impairment. Particularly, patients taking strong CYP3A4 inhibitors and those taking medications that may cause gastroesophageal reflux or esophagitis need careful monitoring. When taking this medication, patients should be cautious as side effects such as dry mouth, constipation, blurred vision, urinary retention, and dry eyes may occur. Especially, dry mouth and constipation become more common with higher doses. In severe cases, serious gastrointestinal problems such as fecal impaction or intestinal obstruction may develop. This medication may cause drowsiness or fatigue when driving or operating machinery, so caution is required. Additionally, patients with prolonged QT syndrome or hyperkalemia may be at risk for cardiac arrhythmias, so caution is needed. If severe allergic reactions such as angioedema or anaphylaxis occur, immediately discontinue the medication and seek appropriate treatment. Safety during pregnancy has not been established, so use with caution in pregnant women, and breastfeeding women should preferably avoid this medication. Safety in pediatric patients has not been confirmed; therefore, it should not be used in children. In the event of overdose, symptoms such as headache, dry mouth, dizziness, drowsiness, and blurred vision may occur; in severe cases, professional treatment may be necessary. Inducing vomiting in the case of overdose should be avoided, and administration of activated charcoal or other appropriate treatments should be undertaken. This medication may enhance the effects or worsen side effects when taken together with other anticholinergic drugs, so caution is needed. It may also reduce the effects of medications that increase gastrointestinal motility. This medication is metabolized by certain enzymes (CYP3A4) in the liver, so when taken with drugs that strongly inhibit this enzyme, dose adjustment is necessary. Conversely, when used with drugs that induce this enzyme, the effect of the medication may be reduced. Finally, before taking this medication, the cause of frequent urination should be verified for other conditions, and appropriate treatment should be received for urinary tract infections if present.
Not everyone experiences these, and they can vary from person to person.
This medication is sensitive to air and moisture, so it must be stored in a tightly sealed container. It is best to maintain the storage temperature between 1 degree and 30 degrees Celsius.
