Summary

Main use
Treatment of gastrointestinal diseases such as gastric and duodenal ulcers, gastritis
Who takes it
Adults
Adult dose
Typically, irrespective of meals, 2 tablets are administered orally twice daily, in the morning and before bedtime. 1) Gastric and duodenal ulcers 2 tablets are administered twice daily, or 4 tablets are taken once daily before bedtime. In particular, administering 4 tablets twice daily, in the morning and at bedtime, for duodenal ulcer patients yields a higher healing rate. Dosage remains the same for ulcers caused by NSAID administration or in combination for the prevention of gastric and duodenal ulcers during long-term NSAID therapy. Generally, conditions due to NSAID administration heal within 4 to 8 weeks; however, healing times may vary among patients. For recurrent ulcer patients who showed excellent results with short-term treatment, maintenance therapy of 2 tablets once at bedtime is recommended. 2) Gastroesophageal reflux disease For 8 weeks, take 2 tablets twice daily or 4 tablets once daily before bedtime. In severe cases of gastroesophageal reflux disease, the dose can be increased to 4 tablets four times daily. 3) Zollinger-Ellison syndrome Start treatment with an initial dose of 2 tablets three times daily; it can be increased as needed. For severe cases, as instructed by a physician, doses of up to 6 g daily can be administered. 4) Peptic ulcers, acute stress ulcers, and upper gastrointestinal bleeding due to acute mucosal lesions Once oral administration is possible, switch from intravenous to oral administration, taking 2 tablets twice daily. 5) Pre-anesthetic administration (prevention of Mendelson's syndrome) 2 tablets are to be taken 2 hours before anesthesia induction. It is also beneficial to take 2 tablets the night before surgery.

Precautions

This drug should not be used in patients with severely impaired kidney function or those undergoing dialysis. It should also not be administered to patients who have a history of allergy to this drug or its components. Patients with a history of acute porphyria, a rare blood disorder, may experience seizures and should not receive this medication. It is contraindicated in patients with kidney impairment, those who have undergone gastrointestinal surgery, pregnant or potentially pregnant women, and premature or newborn infants. Additionally, it should be avoided in patients who need to restrict potassium intake. Caution is advised in patients with impaired liver function, patients with a previous history of drug allergies, elderly patients, and patients with a history of kidney problems. Patients taking NSAIDs, especially elderly patients or those with a history of peptic ulcers, should have their condition monitored regularly. This medication can mask symptoms of gastric cancer, so patients with gastric ulcers or those who have recently developed or changed dyspeptic symptoms above middle age should confirm that it is not a malignancy before administration. Patients with Helicobacter pylori-induced gastric and duodenal ulcers require treatment to eradicate the bacterium. In critically ill patients, this medication may cause the formation of bezoars after administration; caution is needed. Patients with hypophosphatemia should be careful as the aluminum component of this drug may interfere with phosphate absorption. In patients who have difficulty swallowing, respiratory complications have been reported after taking this medication, so it should be used cautiously. Rarely, during treatment with this medication, serious allergic reactions, such as anaphylactic shock, can occur; if symptoms appear, administration should be stopped immediately and treated. Allergic symptoms such as skin rash, hives, angioedema, fever, bronchospasm, hypotension, and chest pain may also occur infrequently, and if these symptoms appear, medication should be discontinued. Blood tests may reveal abnormalities, including leukopenia and thrombocytopenia; if systemic malaise, weakness, bleeding, or fever occurs, urgent testing is recommended, and the medication should be discontinued if necessary. Impaired liver function may occur; therefore, if jaundice or symptoms of hepatitis are apparent, the medication should be discontinued. Rare cases of cardiac arrhythmia or vasculitis have been reported, so attention to cardiovascular symptoms is essential. Gastrointestinal symptoms may include nausea, vomiting, constipation, diarrhea, and abdominal pain; stool color may change, but treatment is not required. Severe symptoms such as dizziness, drowsiness, confusion, seizures, insomnia, headache, hallucinations, and neurological symptoms may occur, particularly in severe and elderly patients. Myalgia or arthralgia may occur rarely, and if severe muscle damage symptoms appear, medication should be stopped immediately. Skin rash, hair loss, and pruritus may occur infrequently. It is advisable not to use this medication continuously for long periods; if there is no therapeutic effect, a change to another medication should be considered. Elderly patients, those with chronic pulmonary diseases, diabetes, and immunocompromised individuals may have an increased risk of pneumonia following this medication, necessitating caution. If dizziness or drowsiness occurs during treatment, driving or operation of hazardous machinery should be avoided. This medication may interact with other drugs, so consultation with a physician or pharmacist is necessary, particularly when taken with anticoagulants, antibiotics, or anticonvulsants. It is safest for pregnant or potentially pregnant women, as well as nursing mothers, not to use this medication. Safety and efficacy have not been adequately established for children; thus, it is preferable not to give it to them. In cases of overdose, emergency treatment such as vomiting or gastric lavage may be necessary, and hemodialysis can be performed to eliminate the drug. This medication can decrease gastric acid secretion and promote bacterial growth in the gastrointestinal tract; therefore, ocular examinations may be needed during long-term use.
